A caregiver is responsible for supervising a group of infants ranging from 5 months to 17 months with a specified age-group of 11 months. The ratio for 0-11 months is 4 to 1. The deficiency was corrected during the inspection when the second caregiver returned to the classroom.

During the inspection, 2 pre-k classes are not served meat/protein alternative during lunch. The menu did not list a protein served for the day. The meal patterns established by the U.S. Department of Agriculture Child and Adult Care Food Program are not followed.

A careegiver lacks 24 annual training hours from September 2022 to September 2023. A caregiver lacks 22 annual training horus from November 2022 to November 2023. A caregiver lacks 21 annual training hours from September 2022 to September 2023. A caregiver lacks 4.5 annual training hours from April 2023 to April 2024. A caregiver lacks 19 annual training hours from August 2022 to August 2023.

A child did not have a health statement on file.

The operation does not have a current gas inspection.

A child did not have shot records or an immunization exemption form on file.

A driver does not have current transportation training.

A child was observed napping in front of an entrance and exit door. Placing a child to nap in front of an exit door poses a risk of a child being walked on and injured during an emergency evacuation. The director immediately moved the child in an appropriate nap area in the classroom.

Based on the interviews with staff, the operation was found to been out of compliance with the minimum standards for a licensed childcare center. 4 young children were left in a classroom unsupervised.

Management did share an incident report with the parents of 4 children of an incident that placed that children at risk. The children were left in a classroom unattended.

The operation did not notify 4 children parents of a situation that placed them at risk at the time of the incident. The children were left unattended in a classroom. The deficiency was corrected during the inspection when the director contacted the parents by phone.

Based on a DFPS investigation, it was determined that management did not immediately notify the parent of a child that was in a situation that placed the child at risk.

Based on a DFPS investigation, it was determined that management did not report to Child Care Regulations a situation that placed a child at risk within 2 days of the occurrence.

Based on a DFPS investigation, it was determined that a young child had access to medication.

Based on interviews with the staff and the parent, a caregiver observed a young mobile infant with a pill in the child's mouth as the child was crawling during floor time.

Based on documents provided, the operation did not maintain a daily report on ,4/23/2024, to share with a young infant's parents.

Two children did not have the required vision and hearing screenings on file for review.

The operation's last documented fire inspection by the local Fire Marshal was on 4/3/21. No other record of a recent inspection was available for review.

At the time of inspection, five children enrolled into care full time did not have their most current, based on their age, immunizations on file available for review.

Based on a review of the information a caregiver at the operation did not deny that she yelled at the children.

Caregivers, in charge of children under 24 months, were lacking the training in early brain development.

Staff were missing the required training in building/premises safety and handling/storing of hazardous materials.
