Four children's files reviewed; one is missing and a second one is missing health statement.

Three out of four children's files reviewed were not available at the time of inspection.

Provider acknowledged that caregiver will not return until eligibility notice is requested and received.

Records for dog residing at the operation indicate that rabbies vaccine expired 10/22/2024.

Operational Policies were reviewed and are missing the following information: (4) Procedures for handling medical emergencies; (8) Promotion of indoor and outdoor physical activity including (a) duration (b) recommended clothing and (c) plan during days with extreme weather; (9) Parent's rights; (10) Instructions on how parent may access (a) Minimum Standards Online (b) Texas Abuse/Neglect Hotline and (c) HHSC website; (11) Emergency preparedness plan.

A tree stump was observed with mushrooms dark brown/black in color within children's reach.

The diaper changing table being used was made of an absorbent and hard to clean fabric.

The director has not been able to obtain the statement of health from a local veterinarian for a small dog that is present at the home.

The director did not have current training in Pediatric first aid with rescue breathing and choking; and Pediatric cardiopulmonary resuscitation (CPR).

The Children's Products Certification form was not completed to prove that the director has reviewed each of the recall notices issued by the CPSC and that there are no unsafe products in the home.

There was no statement of health from a local veterinarian for a small dog present at the home.

There were no records available for review for three children in care.

The operational policies reviewed did not include information about the presence of a dog inside the home.

The mats where children were laying on were not labeled with the child's name or numbered. Note: This was CORRECTED during the inspection as the mats were labeled with a number and a number/child assignment map was created.

Fire drill practices were being conducted every three months instead of monthly.

The director was caring for a total of 12 children. In the group 10 of those children were between the ages of 18 months - 3 years of age. According to the ratio chart the maximum number of children one caregiver may supervise between ages 18 momths-3 years is 8.

The Children's products certification Form was not current. The date on the form provided was 2018.

The director did not have current training in Pediatric first aid with rescue breathing and choking; and Pediatric cardiopulmonary resuscitation (CPR). The expiration date on the certificate of training reviewed was January 2020.

The director was missing 30 clock hours of annual training for the year of 2/24/2020-2/23/2021.

There was no file available for review for a child that was in care. Children's records reviewed were missing the following information: Two files were missing the Authorization to obtain emergency medical care and to transport the child for emergency medical treatment. Two files were missing the child's health statement. One file was missing the child's immunization record.
