Two out of five staff training records reviewed are missing certificates for the following trainings: -Hazardous Materials -Emergency Preparedness

Four staff trainings records were reviewed. Some of the training certificates for three of files were not available for review, including trainings on the following topics: -Hazardous Materials -Administering Medications -Emergency Preparedness -Building and Physical Premises Safety

A caregiver was caring for six children of varying ages including infants. The children's ages were three 17 month old infants and three toddler aged children. The specified age of the children in the group was 17 months. According to the ratio chart a caregiver may care for a maximum of 5 children.

The Childrens Products Certification form used for verifying that the opreration reviewed the United States Consumer Product Safety Commission (CPSC) recall list was not current. The date on the form was 7/18/2022.

Most recent fire inspection provided was dated 03/14/2023 and a renewal inspection is due every twelve months.

A caregiver was missing 11 clock hours of annual training for training year 4/27/2023 - 4/26/2024.

1 of 6 children's records reviewed was missing the child's statement of the child s health from a health-care professional. 1 of 6 children's records reviewed was missing Name, address, and telephone number of another responsible individual (friend or relative) who should be contacted in an emergency when the parent cannot be reached. 1 of 6 children's records reviewed was missing the parents authorization to obtain emergency medical care and to transport the child for emergency medical treatment.

The director did not have training in transportation safety.

The children's sign in and out log was not maintained. Only one of 13 children in care was signed in. NOTE: This was CORRECTED during the inspection when all children in care were signed in.

A fire drill was not practiced for the month of April 2024.

A caregiver was missing 7 clock hours of pre-service training.

One employee had an expired CPR/First Aid Certificate.

One 11 month old infant was observed to be in care with a 4 year old.

An 11 month old was observed to be sleeping in a playpen with a blanket.

A caregiver was observed caring for 5 infants in the infant room. The specified age of the children in the group was 0-11 months. According to the ratio chart the maximum number of children a caregiver may supervise is 4.

The children's products certification form used as verification that the center has reviewed each of the recall notices issued by the CPSC and ensured that there are no unsafe products in the center was not current. The date documented on the form was 12/07/2020.

A caregiver's record was not available for review during the inspection. A caregiver was missing 9 clock ours of annual training.

The diaper changing mat in the infant room was observed torn.

A caregiver was caring for a total of 12 children of varying ages in the toddler room. The specified age of the children in the group was 2 years of age. According to the child/caregiver ratio chart the maximum number of children a caregiver may supervise is 11.

The center did not have a current annual fire inspection. The fire inspection report reviewed had a date of 12/2/2020.

An inflatable bounce house that was not in use was observed in the children's outdoor playground. The bounce house was not properly stored and presented a tripping hazard for children. Note: This was CORRECTED during the inspection as the inflatable bounce house was removed from the play ground and stored away.
