The required Consumer Product Safety Commission form was not available for review. This was corrected when the director printed it out and signed it.

The director's SIDS training was not available for review.

The plastic and wicker containers in several rooms that hold toys were damaged, leaving rough edges and sharp metal that could pose a risk of injury to children in care. This was corrected when the containers were removed.

Based on information gathered during a DFPS investigation, it was determined that caregivers were not engaged in active supervision of children in care, resulting in one child touching another child inappropriately on their bare bottom.

One out of five infant teachers present during this inspection had an expired SIDS training certificate. The training was last completed in May 2023.

A stall in the boy's bathroom had peeling paint on the wall.

Four highchairs in the older infant room were not equipped with safety straps. Three infants were observed sitting in the highchairs not strapped in. This was corrected when safety straps were found and put on the highchairs.

A sheet that was brought from home did not fit the mattress. Several other sheets did not fit the mattresses correctly causing the mattress to curl up. This was corrected when the caregiver tightened the sheet up.

Based on the inspection visit on May 7th, several rooms become over ratio by one very quickly where the director had to step into a classroom. The director was proactive by taking a few children with her to go to classrooms to move children around.

Based on video evidence reviewed as a result of this investigation, a child raised their head from the nap mat as though attempting to get up and the caregiver used her hand on the child?s back in a rubbing motion to push the child to lay back down.

The last fire inspection was conducted on 5-19-2022. Fire inspections are due annually.

Five out of ten children's records did not have a health statement signed by a health care professional after being enrolled at the center for over a year.

A staff that was hired a week ago and was shadowing the Pre School teacher was present at the operation, prior to receiving the eligibility notification from the background check unit. This was corrected when the director removed the person from the room.
