A caregiver that provided transportation for school pick-up had expired transportation safety training. The training expired on 4/15/2026, and a child was left on a van 5/1/2026.

The alarm on the van was not maintained, and quit working, therefore a child was left on the van and unattended for approximately 80 minutes.

A caregiver did not have a list of the children for the van she was driving. At school pick up, four children were put on her van, but she did not know which children they were and had no list to check or confirm.

A child in care was left on a van, in the garage, after school pick-up. The caregiver unloaded the children from the van not realizing a child had fallen asleep, leaving the child unattended in the van. The caregiver did not walk the van to ensure all children exited the van. The child woke up, got out of the van, got out of the garage, and had to cross the street, alone, to get to the daycare building, where the child was found.

A child in care was left on a daycare van after school pick-up. The caregiver parked the van in the garage, not realizing there was a child asleep. The caregiver did not have auditory or visual awareness and was unable to ensure the child's safety and well-being, the caregiver did not realize the child was missing from the group.

A caregiver was unaware which children she was responsible for at school pick up. There were 9 children, four children got on the van, and five got on the other van; however, the caregiver did not know which of the children got on her van. This resulted in one child being left on the van once they returned to the daycare.

An infant less than 12 months of age was observed at inspection asleep in their crib with a blanket. It was correceted at inspection with the blanket being removed out of the crib.

A child in care was noticeably upset after morning drop off and the caregiver did not attempt to verbally comfort the child or physically hold the child to comfort the child.

Based on the evidence obtained during the investigation, caregiver transitioned their class into the building from the playground and left a child unsupervised outside for 20 minutes.

The younger toddler playground had a couple rocks that are the size of a large marble that two different children found one and placed it inside their mouth. Additionally, the playground has bird feathers throughout the play space that need to be removed. The caregiver removed the rocks from the children's mouth and disposed of them.

A caregiver was observed at inspection using an abrastive tone with the children when entering from the playground and lacked patience and respect in her communication with her group of children.

The most recent health inspection was conducted over 12 months ago and was due by 05/11/2024.

Toddlers are not provided enough variety of activities, including low shelving for easily accessing the selection of toys during the morning drop off time. Additionally, Pre-K age children lack available interest centers.

An infant, less than 12 months of age, had a small lovie blanket in the crib while asleep and it was covering the infant's face. It was removed from the crib at inspection.

Children did not wash their hands in preparation for morning snack.

Staff did not have a list with the children's names and ages and were not knowledgeable about the ages of each of the children they were accountable for.

Infants and older children were combined with a total of 18 children with a 30 month age gap between the youngest infant and the oldest child.

A classroom that was used for toddlers was changed to be added for infants. The infant classroom previously has been closed and no longer used. Space was changed without a notification to child care regulation. It was corrected at inspection with child care regulation being notified and the added room evaluated for compliance.

The added infant classroom only has one hand washing sink available and it is also used for bottle preparation.

Four staff personnel records was lacking various topics related to health and safety, including preventing and responding to food allergies, medication training and building and premise safety training.

A food allergy plan for a child in care with a diagnosed food allergy lacks the physician signature.

An infant in care was eating in a high chair and the buckle was not fastened after placing the child in the high chair.

Two caregiver records was lacking documentation of pre-service and orientation training. Additionally, orientation was not completely conducted within 7 days of the employment date. One staff member is lacking orientation and a notarized affidavit of employment.

All three first aid kits used in the vans to transport children in care had expired antiseptic wipes. Additional antiseptic wipes that is not expired was placed in the kits at inspection.

Older infants were observed secured in the multi occupancy tables for over 30 minutes prior to meal time and stayed in the seats an additional 30 minutes after meal time.

Infants were observed at inspection having pretzels served on the bare table without a napkin or plate.

A caregiver of the operation does not have an eligible notification for one employee that was re-hired on 8/4/22.

A caregiver sent a child outside of the classroom to a neighboring classroom to complete job tasks, such as turning on the classroom light and sound machine, one two separate occasions. While the child was in the neighboring classroom the caregiver did not have visual or auditory awareness, was not aware of the on going activities and was not within physical proximity. The caregiver was unable to adequately supervise all of the children she was responsible for.

A one month old infant was asleep on a boppy pillow on the floor for over an hour and was not moved to their crib by either of the two caregivers available in the classroom.

During naptime, one classroom had one caregiver with 13 children, ranging in age of 17 months to 2 years of age. With the classroom having one caregiver within the group, the classroom is unable to reduce to naptime ratios and were over ratio with a specified age group of two year olds.

The operation re-hired two previously employed caregivers at the operation and did not re-submit their background check prior to having the caregiver supervising children in care. Background checks were re-submitted at inspection and staff left the operation.

The most current annual health inspection at inspection was conducted on 1/26/2021 and was due by 1/26/2022.

Diaper changing was observed with the infants classroom and multiple diaper changing was observed and none of the infants had their hands washed after diapering.

Based on interviews with staff, the infants are not having morning and afternoon opportunities daily to be outside. Additionally, the infants only go outside maybe once a week and does not include two daily opportunities in their written activity plan.

Ten personnel records did not have documented orientation. One record was lacking a photo identification. One record was lacking a notarized affidavit of employment.

Based on the evidence obtained during the investigation, a child in care was being discipline with being in time out for longer than ten minutes for a pre-k age child.
