A live ant bed was observed on the playground.

Six students were observed to need updated health statements.

One staff file evaluated was missing at least one clock hour of the annual training hours must focus on prevention, recognition, and reporting of child maltreatment.

One toliet is not functioning gushing water out of the pipe when flushed.

Two staff members are needing four hours and two staff member is needing six hours in one or more of the following topics: child growth and development; guidance and discipline; age-appropriate curriculum; and teacher-child interaction.

Two students were observed to need updated immunizations records.

Five staff members are needing annual trainings that include emergency preparedness, preventing and controlling the spread of communicable diseases, administering medication, preventing and responding to emergencies due to food or an allergic reaction, understanding building and physical premises safety, and handling, storing, and disposing of hazardous materials.

An individual that did not have a background check completed was present at the operation working with the children.

A child was left unsupervised in the classroom.

The diaper changing sink in the infant/toddler changing area was being used for purposes other than hand-washing. This was corrected when a conversation was had with the director and the caregiver and the other items were removed from the sink.

The fire extinguishers were not inspected in the month of July. This was corrected when the director inspected the fire extinguishers and recorded it on the emergency practices form.

The carbon monoxide detector was not checked in the month of July. This was corrected when the director checked the detector and recorded it on the emergency practices form.

An expired epi-pen was stored with the medications at the operation. This was corrected when the epi-pen was set aside to give back to the parent at pick up.
