According to documentation the operations fire inspection was conduced on 2/26/2025 and the operation stated that had an isnpection completed but did not have the report avaliable for review.

Cots in four of the classrooms were not labeled.

Infant feeding sheets in two infant rooms were not updated.

The operation made a change in Director on 3/2/2026 and did not notify licensing.

A caregiver was observed popping a child on the hand and on the back of the head.

A toddler was left unsupervised for three minutes on the playground. The caregiver realized that the child was missing when they returned to the classroom.

A staff member failed to supervise children appropriately on the playground. A staff member was unaware that children were in a blind spot on the playground which resulted in them opening and closing the gate. The children were observed walking out of the gate. The children were observed using the playground equipment inappropriately.

This standard was evaluated as part of an investigation and it was determined that a caregiver was pulling and grabbing children inappropriately to get them to lie down on their nap mats.

During an investigation it was determined that the caregiver failed to disinfect the changing table between diaper changes.

This standard was evaluated as part of an investigation and was determined that a caregiver was being rough with children. The caregiver was seen on video grabbing and pulling children, forcefully putting them on their nap mats, pushing a child's face into the mat and picking children up by their forearms and carrying them across the room.

This standard was evaluated as part of an investigation and it was determined that a caregiver witnessed another caregiver abusing children and did not report it to Child Care Regulation or notify the director.
