Based on the information gathered during the investigation, it has been determined that the school failed to report immediately to the parents that a child had been involved in any on-routine situation that placed, or may have placed, the child at risk for injury or harm when released to a person that was not designated to pick up. The child was out of operation's supervision and with this undesignated person for 17 minutes plus 3 minutes in addition since the school changing child. A total of 20 minutes passed before parents were contacted.
