The parents rights were missing from the parent handbook also called the operational policies. This was corrected at inspection.

The data test for the turf was not available for review during the visit.

The cleaning supplies were accessible to children in 2 out of 10 classrooms.

One of the director's last day was on 10/1/24 (per controlling person submission). No notification in writing to licensing was completed.

Two out of ten files did not have trainings up to date.

Two children do not have food allergy plans signed by the parents or the health care professionals.

This standard was evaluated as part of an investigation. Based on information obtained from observations from video footage, a caregiver was observed aggressively grabbing a child by both arms, picking the child up, and sitting the child forcefully down on the floor. The caregiver was observed to grab the child again to separate them from the group and then forcefully sit the child on the floor against a wall adjacent to the group.

This standard was evaluated as part of an investigation. Based on information obtained from observations from video footage, a caregiver is observed entering information on an electronic tablet and was not properly supervising children.

Four infants do have feeding instructions signed and dated by the parent or health care professional.

A child was admitted into before their immunization exception was filed and verified by the operation.

One child was admitted to care before meeting immunization requirements.

A caregiver did not remove an infant sleeping in a restrictive device in Infant room I.

Two infants were observed sleeping in cribs that contained toys, blankets and pacificers that were attached to the children by a string or leash.

One caregiver was observed supervising 6 infants under the age of 11 months.

Children were observed in areas of the operation unattended.

A caregiver did not wash their hands after assisting three toddlers with toileting.

Four children under the age of 13 were in care at the operation who were not enrolled.

Several children with known food allergies did not have a completed Food Allergy Emergency Plan in their file.

The operation has incurred several high weighted deficiencies that put children at risk. The infant and toddler caregivers are instructed by management not to take small children outside.

An infant was placed face down and swaddled in a crib.

A cargiver in Infant I was left unsupervised without CPR training.

A caregiver without Pediatriac First Aid was left unsupervised in the Infant I classroom.

Two infants under 5 months old were observed in restrictive devices with bottles propped up with blankets to feed them.

Two children did not have immunization records in their file at the time of inspction. Two children in care did not have the required health statements signed by a physcian. One child did not have required information for a school aged child on their admission form.

A school age child in care was observed walking freely into the building alone, to get a wet paper towel. Inspector escorted the child to his teacher outside.

A crib in the Infant 2 classroom was observed to have an additional supporting mattress.

A caregiver did not use good judgment when they gave one infant another infant's formula.

The operation did not maintain privacy for a child with documented food allergies. This deficiency was corrected at inspection when the Director removed the posted food allergy list and placed it in an area marked Food allergies where staff and parents can view it at any time.

A rotating play structure on which a child sits does not have a use zone of a least six feet.

Three classrooms did not have trash can lids on garbage cans which contained food items.

The operation did not have food allergy plans posted in all classrooms. The operation did not maintain privacy for children with documented food allergies.

An initial background check was not completed for a caregiver who was caring for children during the inspection. This was corrected during the inspection as the caregiver went home. A background check was submitted for the caregiver during the inspection.

This standard was evaluated during the investigation and was found to be deficient. The caregiver used a prohibited punishment on a child in care.
