Observed a caregiver on their phone while caring for children. This was corrected when the phone was put away

Observed a bottle sitting on the changing table. This was corrected when the bottle was removed from the changing table.

Observed medication stored next to food products in the refrigerator. This was corrected when the medication was removed.

Observed the children in two classrooms standing up to eat. This was corrected when chairs were given to the children.

Observed children eating without have water available during their meal. This was corrected when water was given to some classrooms and Licensing spoke with the operation staff about supplying drinking water with each meal.

During a DFPS investigation it was found that there was pealing paint. It was also found that a fence had a large whole in the fencing and another part of the fence was down presenting a risk for children to leave the playground.

A child who is 13 years old has been present at the operation but is not enrolled into the program.

The operation could not show proof that a fire drill was completed in the year 2026. A fire drill was completed during the inspection and the children and staff were able to exit the building in under 2 minutes.

The operation could not show proof of a severe weather drill being completed in the year 2026.

Observed peeling paint in the bathroom, and classrooms. There are cubbies that have visible dirt in them. The sinks in the bathroom looks visibly dirty. The bathroom floors have build up around the walls, The walls in the classrooms have missing paint in sections of the walls.

The sinks in the bathroom looks visibly dirty. One classroom is being used for storage and is not able to have children in the room.

Observed 3 employee files, 2 files did not have affidavits for review.

Observed a caregiver on their phone while caring for children. This was corrected when the caregiver got off their phone.

A caregiver stated that they had 13 children but when licensing counted there were 14 children in the classroom. I explained to the caregiver they must know how many children they have.

Observed the railing of the stairs broken and rotted wood around the water fountain. There was also a PVC pipe that was broken and had sharp edges.

The operation had a tree that had been cut down and will be using the stumps as natural seats for the children. The stumps are laying sideways and are hazards for the children if they roll of a child gets pinched in the logs.

Observed 8 children files, one file was missing an immunization record and a health statement. I spoke to the director about the missing items.

Observed one caregiver who did not have 2 years of experience that did not have preserrvice training prior to working with the children.

Observed 8 children files, two children were missing the emergency Authorization information, one child was missing immunizations and two children were missing Health statements. There were two children who did not have the receipt of operational policies checked.

Obseved 2 outlets without covers. This was corrected when outlet covers were placed in the outlets.

The operation was made aware of a report of innapropriate discipline at the operation and did not report to licensing.

A caregiver came into the operation leaving a caregiver outside with 11 children ages from 18m-2 years old. This was corrected when the caregiver came back outside.

Observed a can of disinfectant spray in an unlocked cabinet assessable to the children in care. This was corrected when the chemicals were removed.
