An epi pen was not in its orginal container.

There was an open and exposed electrical wire in the reach of the children.

The pea gravel loose fill was not maintained at 9 inches around the large play equipment. The gravel measured at less than 2 inches in front of the slides and under the platform.

The use zone for the sitting bounce equipment did not have a 6 foot use zone and had a helicopter in the use zone.

There was a slide on the small playground that was only 4 feet from the fence.

This standard was reviewed in relation to the reported allegations and was found to be deficient. The incident occurred on June 26 and was not reported to Child Care Regulation until July 17th.

One caregiver did not have food allergy training.

Operational policy does not have safe sleep section.

None of the 8 staff members had an emergency preparedness training.

A caregiver, with direct access to children, was present without a background check eligibility. The caregiver was sent home during the inspection.

Operation has not conducted a fire drill since August 23. Fire drill was conducted during the inspection.

An infant who cannot roll over on their own was seen sleeping on their stomach. This was corrected at inspection when the infant was awakened from the nap.

There are two doors that have holes in them and are acessible to children.

Two caregivers did not know the number of children they were in charge of. This was corrected when the caregivers explained a child had came in late and were able to identify which child.

Five of ten staff were observed to have online CPR training. The staff will complete an in person CPR training course to come into compliance.

It was observed that cabinets containing cleaning chemicals were unlocked and accessible to children in each classroom. This was brought into compliance during the inspection by utilizing the cabinet locks installed.

A child made an outcry of being choked, leaving a mark and/or bruise on the child's neck.

It was found that the COVID restrictions were still in place and parents were not allowed to enter the child-care center during operating hours.

It is alleged that a child was abused by caregiver.

It was found that children were being placed in an infant room as a method of discipline.

it was observed that four infants ages 6-9 months were left unsupervised in a classsroom with one 7 year old child when the caregiver left the classroom to use the restroom. It was also observed that one classroom of 12 two year olds was left alone unsupervised during nap when a caregiver answered the door and was walking classroom to classroom with the licensing inspector.

One child of a caregive was observed napping in a PreK classroom. There was no enrollment information on file for the child.

Two of six children's files reviewed did not have physician signed health statements.
