During the walk through, there were two children in separate settings observed unsupervised. One was sitting in the dining area, and the other child was on the Pre-K playground.

One child was missing emergency medical information in their file.

One children did not have immunization available for review.

The director did not have current first aid/cpr certification.

Infant classroom: there was an plastic octupus hanging dryer with clips in a bin underneath the crib that was at hands reach of infants. In the same classroom there was a pushcart that had a broken bobble figure that was detached from the coil, which was exposed. It was corrected at inspection when the equipment/toys were removed from the room. Pre-K Playground: There is a storage trunk on the Pre-K playground that did not meet safety standards with the latch and the breathing gap/hole. This was corrected at inspection when the trunk was removed from the play area. Pre-K playground had some stepping pads made of concrete which two of them had sharp objects protruding out. There is a metal square attached to one of them that had sharp corners.

The operation did not have a current annual fire inspection. Corrected at inspection when fire marshal arrive to conduct inspection and provided a pass fire inspection.

One children was missing their health statement on the enrollment form.

One child was missing the address on the emergency contact person.

The operation failed to report to licensing within two days of receiving parent concerns regarding inappropriate discipline.

A 17-year-old caregiver was supervising children alone in the school age classroom when an incident occurred that cause the caregiver to remove a child from the group by lifting a child from underneath their axillae. .

A caregiver admitted to mocking a child on their physical appearance during a verbal altercation with a child.

The director did not have current SID's or Shaken Baby Syndrome training.

There were cleaning solutions next to the infant formulas. Note, this was corrected at inspection.

There was an infant sitting on a baby walker equipment in the infant room. Note, this was corrected at inspection.

Information gathered for this investigation established a child was sitting on a portable highchair for an extended period to secure the child?s safety while staff were occupied with parent pick up.

Information gathered for this investigation determined the portable highchair attached to the table was not age appropriate as a child was able to stand and touch the floor while being in the safety seat.
