During the inspection there were outdoor play materials which were broken. Two slides which had been broken off from a play stucture were on the playground, three outdoor play tables had broken pieces, and a wooden playhouse had broken doors with sharp edges and staples coming from it which were sharp.

September 7, 2021 Standard § 746.1203(4) - Responsibilities of Caregivers- Supervision of Children

This standard was tasked over for this investigation in found to be deficient. It was determined that a child was left unattended in his classroom during recess time. The child was found in his classroom by the center director while his classmates were outdoors. The operation now has a name to face check system and transition log that has to be completed throughout the day to ensure that each child is present. This logging system and transitions were observed during the inspection time. There are also new caregivers present in the named classroom, additional supervision training will take place, and children were properly supervised at the time.
