A caregiver in the infant room did not wash their hands or the infants hands after diapering. This was corrected at inspection when the caregiver washed their hands and the infants hands bringing the operation into compliance.

A potty chair in the 2 year old room was not sanitized after use and still contained urine. This was corrected at inspection when the potty chair was removed from the room brining the operation into compliance.

Personnel did not have the required health and safety trainigs.

The grass was overgrown in the playground area.

It was determined throughout the course of the HHS Investigation that the parents were not notified for approximately 3 hours and 15 minutes of their child being left alone outside unsupervised.

It was determined throughout the course of the HHS Investigation that a toddler was left alone outside for a 2-minute duration in the enclosed playground.

One personnel file for a current employee was missing proof of orientation. One former employee file was missing proof of orientation.

An attempt was made to conduct an unannounced monitoring/Plan of Action inspection. The operation was closed. The permit holder/director failed to notify CCR of the closure for the week of Spring Break.

It was determined throughout the course of the HHS Investigation that two young children in care were unsupervised by the caregivers for less than a 25 second duration. This was self-reported by the operation and confirmed through reviewing surveillance footage.

It was determined throughout the course of the HHS Investigation that an 11-month-old child in care was asleep in the crib with a tight-fitting bib secured around the infant's neck. There was also a pacifier clip and string that was observed around the infant's neck while the infant was sleeping.

It was determined throughout the course of the HHS Investigation that a caregiver placed an infant in care on their stomach to sleep. Numerous infants were observed sleeping on their stomachs in their cribs. This was confirmed through reviewing surveillance footage.

It was determined throughout the course of the HHS Investigation that an infant in care walked out of the infant classroom and into the operation's hallway unsupervised. The infant in care was left in the hallway unsupervised for approximately 2 minutes. This was confirmed reviewing surveillance footage.

It was determined throughout the course of the HHS Investigation that a caregiver supervising infants in care and counted in child/caregiver ratio was on their personal cell phone. This was confirmed through reviewing surveillance footage.

It was determined throughout the course of the HHS Investigation that a caregiver left the side drop gate down on a crib and walked away from the crib while the infant was awake. This was confirmed through reviewing surveillance footage.

One caregiver was left alone with 8 children in care, ages 13 months-20 months with a specified age group of 12-17 months and a 1:5 ratio. This was immediately corrected during the inspection when another caregiver arrived from lunch, placing the operation back into compliance.

It was determined throughout the course of the HHS Investigation that a prospective caregiver with a "pending" background status had access to children in care without an eligibility notification, placing children in care at risk of harm.

A volunteer caring for infants and counted in child/caregiver ratio did not have the required 8 hours of Pre-Service Training.

During the unannounced investigation inspection files were audited. One caregiver working in the classroom did not have the required annual training.

During the unannounced investigation inspection there were two individuals present in the operation without eligible background checks. This was immediately corrected when both individuals left the operation.

Two volunteers did not have a file of the required paperwork. One caregiver and one operational employee did not have proof of CPR First Aid to review.

It was determined throughout the course of the HHS investigation when reviewing surveillance footage that multiple caregivers were on their cell phones while supervising young children in care on the playground.

The caregiver, director, or designee failed to check the outdoor playground area for hazards prior to children in care having access.

It was determined throughout the course of the HHS investigation toxic mushrooms were located on the playground where young children in care play. The young children in care had access to the toxic mushrooms.

It was determined throughout the course of the HHS investigation through surveillance footage that multiple caregivers failed to adequately supervise young children in care while they were on their cell phone. The lack of supervision subsequently led to a child in care consuming a mushroom while on the playground.

2 of the 8 immunziation records audited did not have a current immunization record on file.

One caregiver failed to interact positively with two children in care by using excessive force.

Three young children in care were left unsupervised by a caregiver for a duration of approximately 3 minutes.

The operation staff failed to report an incident within the required time frame. The incident occurred on 9/26/23. The incident was not reported to Child Care Regulation until 10/5/23.

Infant feeding instructions was current for 1 out of 8 infants in care enrolled.

It was determined that 2 caregivers were watching children in care with pending background checks. The caregivers were allowed unsupervised access to the children in care. This was immediately corrected by removing the caregivers from the classroom, placing the operation back into compliance.

During the unannounced inspection multiple holes in the ground were observed on the playground presenting a tripping hazard for the children in care.

The annual fire inspection was not available for review. The last annual fire inspection available was for 6/22/2021.

During the inspection one of the infant classrooms was missing a smoke detector. The annual fire inspection was not available for review during the inspection.

During the inspection 3 of the 4 personnel records surveyed did not have the required health and safety trainings.

During the inspection it was determined that the operation's transportation bus did not have an electronic safety alarm system.

It was determined throughout the course of the HHS investigation that a caregiver did not have current medication training when dispensing medication to a child in care.

It was determined throughout the course of the HHS investigation that a caregiver administered the wrong dosage of medication to a child in care, making a medication error.

During the unannounced follow-up inspection safety straps were missing from the high chair table in an infant classroom. The chairs that had safety straps were not fastened. This was immediately corrected during the inspection by fastening the safety straps and removing children in care from the high chair table, placing the operation back into compliance.

During the unannounced follow-up inspection a water hose was observed laying on the ground on the playground presenting a tripping hazard. This was immediately corrected during the inspection by removing the water hose from the playground area.

During the unannounced follow-up inspection the infant caregiver stepped out of the classroom placing the classroom out of compliance with a 1:11 ratio with 12-17 months olds.

It was determined throughout the course of the DFPS Investigation that five children in care were not properly supervised by the caregiver. Five of the children in care exited the gate from the playground and walked into the street towards the stop sign on Avenue R for an undetermined period of time.
