At the time of the inspection the 2 vehicles used for transportation did not have working child safety alarms that are required to ensure that caregivers check the vehicle completely, preventing any children from being left inside the vehicle.

At the time of the inspection one of the vehicles used for transportation did not have a fire extinguisher mounted in the vehicle. This was observed laying on the floor in the passenger seat side.

On the outside playground, a piece of equipment was found to have several cracks. There was a crack which posed a pinching hazard to children. Staff were notified of the issue and the equipment was immediately moved from the playground and placed by the trash bin for pickup.

This standard was evaluated in regards to a report and was found to be deficient. The center policy is that each caregiver on the field trip routinely check the written list of children. One caregiver failed to follow procedure and as a result a child was not accounted for during a field trip.
