Foreign education documents have not been translated into English to verify the education meets the requirements of a United States high school diploma.

Four staff did not receive training in or more of the following topics: emergency preparedness, communicable diseases, food allergies, administering medications, understanding building and physical premises safety and handling, storing and disposing of hazardous materials.

Electircal fans in the infant and toddler rooms were not safeguarded or out of reach of children.

The infant cribs in the infant room did not have tightly fitted sheets. The sheets had waves and ripples and could be pinched up by using fingers.

Infant bottles were not correctly labeled and one bottle did not have any name label. This was corrected at inspection.

The most recent inspection form 2936 was not posted in a prominent place.

A 3 month old infant was sleeping in a sleep swaddle with restriced arms without a sleep exception form signed by a healthcare professional. This was corrected when the swaddle was removed off of the baby.

Montly maintenance checklist conducted by the director were not being maintained.

An infant was found sleeping or laying down with a bottle in their crib. This was corrected when the bottle was removed from the infant and crib.

The CPC review was last completed in 2020.

The ouside play area has several concrete holes that have sharp ridges, broken metal, and stone breakage that can be extremely risky while children are engaged in outside play.
