A child sustained an injury while in care as a result of unsafe staff interaction.

Based on the findings from a DFPS investigation, corporal punishment was used on a child in care when the caregiver was popping, swatting, and grabbing the children in care.

During the walk-through in Classroom 350A, The inspector observed an individual accessible to children without a cleared background check. Note: Corrected during inspection. Individuals left the operation. no longer accessible to children.

While reviewing children's files, one child did not have an admission date listed. Note: Corrected during inspection. Updating the file, adding the admission date.

While receiving first-aid kits located in buses, Missing Items: Scissors, thermometers, and tweezers In one first-aid kit, Inspector observed expired burn cream medication. Note: The director removed expired Burn cream medication. and included missing items.

During the walk-through, it was observed that a spray bottle of cleaning supplies (Fabuloso-Purple) was under an unlocked cabinet accessible to children in Classroom 400.--- Note: Corrected at inspection. caregiver locked the cabinet.

During the walk-through of six buses, one bus alarm system was not working properly.

While reviewing the background checks, it was discovered that a caregiver was accessible to children without an approved background check. Note- Corrected at Inspection. The caregiver without an active background was sent home.

Both caregivers were unaware of the ages of the children they were watching during the walk-through in classroom #300.

Based on the information obtained during the investigation there is sufficient evidence to show that the operation was aware that a child sought medical attention for an injury that occurred at the center and did not report this incident to CCR.

I observed three food allergy plans that had not been signed by the child's parent.

I did not observe an address documented for the emergency contact person in 2 out of 10 children's records evaluated. NOTE: This was corrected during the inspection.

I did not observe documentation of current certification in SIDS, Shaken baby Syndrome and Abusive Head Trauma for 2 caregivers who works with infants and toddlers. NOTE: This was corrected during the inspection.
