Five of the six staff files did not have the following required personnel information: Preemployment affidavit

One education document (for a caregiver) written in a foreign language was not translated into English and did not show how it was equivalent to a high school diploma program in the United States

The operation did not have a current fire inspection available for review at the time of the inspection.

The operation did not notify the parent at pick up when a child received an injury.

The director was found to be without a renewed CPR/First Aid training certificate during the inspection.

A disinfectant bottle that was accessible to children was found in an unsecured cabinet during the inspection. NOTE Corrected during inspection. The caregiver removed the disinfectant to a secure cabinet.

I did not observe documentation of the 24 hours of annual for the 2021-22 training year in 1 out of 4 personnel records evaluated.

I observed that the abuse/neglect training was expired in 1 out of 4 personnel records evaluated.

I observed a child being served snack without his hands being washed. NOTE: This was corrected during the inspection.

I observed that the first aid kit did not include cotton balls.
