Four children's files did not have immunization records on file.

One child who has been enrolled for over 12 months did not have a health statement on file.

In Bus 1 the fire extinguisher had not been properly serviced.

Based upon information gathered during investigation, the operation did not notify a parent of a child in care immediately about an incident that may have place a child at risk or harm.

Based upon the information obtained during the investigation, sufficient evidence was able to support the use of inappropriate discipline of a child in care.

Based on DFPS findings and documents obtained, the incident report created by the operation was not signed by the director and/or designated person in charge for an incident that placed a child at risk while in care.

Based on DFPS findings, a self-report to licensing was not received regarding an incident that placed a child at risk for harm and injury.

Based on DFPS information obtained, a caregiver did not demonstrate competency, good judgement, and self-control when addressing a child displaying challenging behavior by placing their hand around the child?s neck leaving a mark.

Based on DFPS investigation information obtained, the incident report created by the operation addressing an incident involving a caregiver placing their hands around the child's neck did not list the required information. The report was missing details on what occurred causing the child to be placed at risk, information on who witness the incident, if any first aid and or medical treatment was required and required signatures.

Based on DFPS investigation findings the incident report created by the operation was not provided to the parent to review and or sign addressing an incident which placed their child at risk for injury/harm when being inappropriately disciplined while in care.

Based on DFPS findings, physical abuse occurred at the operation.

The center did not report the number of employees leaving employment last year.

One out of 5 children records evaluated were missing an health statement.

The director was missing training in the following topics; Preventing and controlling the spread of communicable diseases, including immunizations, Preventing and responding to emergencies due to food or an allergic reaction,Administering medication ,Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Four out of four caregivers were missing training in the following topics; Preventing and responding to emergencies due to food or an allergic reaction,Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Based on the information obtained, 3 children confirmed a child being disciplined/spanked in care. One parent confirmed that they were aware of a child being inappropriately disciplined in care by their guardian. Another parent acknowledged hearing about the allegations.

Based on the information obtained throughout the investigation, I was informed that children cross/climb across the gated outside area to obtain supplies. Once children cross the gated area staff are not able to observe them, which brings about supervision concerns.

A broken bar rail was observed hanging from the fence. The top of the fence has ridged, wired edges that may cause harm to children in care.
