1 of 6 files did not have documentation that the caregiver meets the educational requirements for a caregiver. Caregivers chosen for their knowledge of the developmental needs of the children in their care and for their ability to respond appropriately to these needs promote a healthy and safe child-care environment.

A child with a food allergy plan had a food allergy plan as required but the medication required for the plan was not onsite.

Documentation of the most recent health inspection was not available. Documentation onsite had a date of 1/5/25 which is more than a year ago. Ownership stated a health inspection has been conducted since then but the documentation was not available.

The director was missing the 30 annual training hours.

Four of the four files reviewed were missing the (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Administering medication, if applicable, including compliance with 746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?); (4) Preventing and responding to emergencies due to food or an allergic reaction; (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials

One infant did not have a current signature of the infant feeding form.

Two files reviewed was missing the education qualification.

In the older infant room a caregiver cleaned a child's nose and used hand sanitizer instead of washing hands with soap and water. Note: When pointed out the caregiver washed their hands.

An infant was observed in a crib with a blanket. Note: The blanket was immediately removed.

One infant, present during the inspection, did not have a updated feeding form. The last time the form was updated was 9/2021. Note: A scanned copy of the form was made.
