Based on video footage obtained, one staff member was observed grabbing an infant in an aggressive manner when she moved the infant from one area to the carpeted area and quickly turned the infant's body to the mirrored shelf. The staff grabbed the infant?s left arm in attempt to prevent the child from maneuvering from placement.

There was not food allergy/emergency plan observed for one child enrolled with documented allergies. The child's enrollment form addressed diagnosed allergies, but did not include the health care professional signature, and/or parent signature. The child's enrollment date was 05/28/24.

This standard was evaluated in regard to a report received and compliance was not met. A child fell to the floor as they were getting off the top of a table. The caregiver stepped over the child without assisting, acknowledging or ensuring the child was not injured.

Two staff observed in the kitchen area was not aware of the ages of the youngest child in care. The youngest child in care was introduced as 6 months when the proper age is 14 months of age. The caregiver in the Infant 3 classroom was not aware of the age of the youngest child in care. Operation staff was provided proper ages of children/infants in which they were responsible for.

One staff CPR/First-Aid certification did not adhere to the guidelines of skill-based training or consist of a curriculum that incudes use of a CPR manikin.

A caregiver was allowed to have direct access to the children prior to having a background check ran. Note- This was corrected during the inspection when the caregiver was sent home until the results of the background check are received.

A climbing structure that includes two small slides was not placed on loose fill or unitary surfacing.
