Five children files did not include a health statement.

One child's file did not have an immunization record available for review.

The primary caregiver training records were not available for review. A total of 10 hours were available for review from a previous Director credential course, dated February, 2026.

Three pets observed in the home did not have current vaccinations available for Licensing staff to review.

The Parent Right information was not included in the operational policies.

The indoor and outdoor physical activity information was not included with the operational policies.

The operation's liability insurance was not updated by the anniversary date and the documented date was expired.

The primary caregiver did not have 30 hours of required annual training.

The diaper changing mat that is utilized for diaper changes has several tears.

During the walk through, I observed a blanket in the play yard for two children ages 9 months and 11 months old while they were napping. Note - this was corrected at inspection when the caregiver removed the blanket from the play yard.

The caregiver was not able to provide documentation of sign-in and sign-out logs.

The primary caregivers adult son has been present at the home during daycare hours of operation without a background check.

The children were allowed to watch television during naptime. Note - This was corrected at inspection when the primary caregiver turned off the television.

Four out of ten children records observed did not have current immunization records.

It was observed that the primary caregiver was supervising nine children ages 9 months, 11 months, two 18 months, one 19 months, one 2-year-old and 3 three-year-olds. The caregiver was over ratio by 2 children ages 18 months to three-years-old.

During the walk through, I observed the play yards arranged where the infants had access to reach into each others play yard.

There were no written feeding instructions available for the 9 month old and 11 month old infants that are not yet on table foods.

One out of ten children records reviewed did not have an immunization record.

Two out of six children records observed did not include a health statement. One child's health statement was dated 08/30/2021, which should have been updated by 08/2022. One child's file did not contain a health statement.

The primary caregiver obtained a CPR/First-aid certification via web. There was no documentation for a hands-on course, and/or use of a manikin to properly rehearse/perform hands on skills based on instructions.

The children had not been signed in for the day. Note- This was corrected during the inspection when the children were signed in.

The operation has not been testing the smoke detectors and carbon monoxide detectors monthly. Note- This was corrected during the inspection when the primary caregiver tested the smoke detectors and carbon monoxide detectors.

The first aid kit was missing a thermometer, scissors, gauze, and medical tape. Note- This was corrected during the inspection when the missing items were added to the first aid kit.

One out of four children's records evaluated did not have updated immunization records on file.

Two out of three dogs did not have proof of up to date vaccinations.

The operation did not have a Children's Product Safety Annual Certification. Note- This was corrected during the inspection when the primary caregiver completed a Children's Product Safety Annual Certification.

The operational policies were not available for review.

The children were allowed to watch television during naptime. Note- This was corrected during the inspection when the television was turned off.

The operation does not have a written activitiy plan.

There was no statement of health for a pet ferret in the home.

The operation has not conducted a fire drill in 2021. Note- This was corrected during the inspection when the operation conducted a fire drill.

Two out of four children's records evaluated did not have a health statement signed by a physician.
