Based on the information obtained, there is evidence to show that the operation was informed of medical treatment and did not report it to licensing.

I observed one caregiver with a pending background check to be present in the classroom caring for children.

Based on information obtained, there was sufficient evidence to support a child was left in the hallway unsupervised.

During the walk-through in classroom Pepper, no disposable towels were available near the hand-washing sink. Note: caregivers added disposable towels near the hand-washing sink.

One caregiver, counted in ratio in the pre school A classroom, did not have a active background check. The background check was noted as inactive 3/14/23. Note: The background was resubmitted during the inspection.

In the Pre School 2 classroom and Pre School 1B both had a power strip accessible to the children on the floor that had exposed electrical outlets. Note: A picture was taken.

In the Pre School 2 classroom there was a child holding benadryl cream sitting at the table with other children. Note: When pointed out the caregiver removed the cream.

Per discussion with the person in charge a fire drill was conducted during the month of August 2023 but was not documented. This was corrected when the drill was documented.

In Prepper A and Prepper B there were torn couches for the children to use. As a result of being torn they cannot be properly sanitized. Note: The couches were removed during the inspection.

In Pre School 1B and in the Make Believe classrooms there were trash cans with diapers in them and the trash cans did not have a lid and the diapers were not in individual sealed bags. Note: This was corrected when the trash cans were emptied.

During the initial walk-through, two caregivers gave the incorrect number when asked how many children were in the group.

I did not observe eligibility results on a background check for a caregiver who was observed working with children during the inspection. NOTE: The caregiver left during the inspection.

I reviewed the center's transportation binders. I did not observe documentation of emergency contact information for some of the children on the list.

I observed that there were not cotton balls in the first aid kit.

I observed the infant feeding instructions had not been updated in the last 30 days for 6 infants who are not yet eating table food.

A caregiver who was counted in ratio walked out of the classroom to put away cots after naptime, leaving one caregiver with 10 older infants. The required/childcare ratio for older infants is 1 caregiver to every 5 infants.

I observed a caregiver in the young infant classroom change a child's diaper without washing the child's hands afterwards.

Ten children's enrollment records were evaluated. The following ws observed: 1. One child'd file was missing documentaiton that immunizations are current. 2. One child's file was missing documentation of vision and hearing screening.

I did not observe documentation of pre-service training in 1 out of 5 personnel records evaluated.

I observed that the food allergy emergency action plan was not signed by the parent.

I observed that the transportation safety training for an employee who transports children expired in August of 2021. NOTE: This was corrected during the inspection.

I did not observe the policy concerning the benifit of activity included in the center's operational policies.

I observed that the first aid kit on one of the busses did not have tweezers. NOTE: This was corrected during the inspection.
