Trinity Lutheran School Extended Care

One child, 2 years of age was left unattended approximately, 1-2 minutes on the playground/garden sensory area. The child entered the building and was located by another staff member.

Staff/caregivers observed caring for the infant age group did not have training related to SIDS/Shaken baby syndrome.

The director is missing training related to SIDS/Shaken baby syndrome.

Five out of five staff records did not obtain the total required 24 annual training hours. Missing training topics include: 1. Emergency preparedness 2. Growth and development 3. Food Allergies 4. Understanding building and physical premises safety 5. Hazardous materials

One child, three years of age was left unattended wandering the hallway of the childcare center. Based on the information obtained the child walked from the Toddler playground area, into the building, and out the front door of the childcare facility. The child was located by the primary caregiver.

One out of three staff observed in the infant classroom did not have an active background check. Reportedly, the staff is a part of the exempt childcare program. Staff left the classroom, in which two caregivers with active background checks were observed in care.

Two out of twelve caregivers CPR/First-Aid certification did not adhere to the guidelines of skill-based training. One staff CPR certification was dated 09/16/2020 and expired 09/2022. The second caregiver obtained online training, dated 08/09/2021 and expires 08/2023.

One caregiver was observed in Room 110 with a pending background check. The caregiver start date is documented as 1/04/2023.

The operational policies do not contain statements on Safe Sleep for Infants, a Comfortable Place for a Mother to Breastfeed, and the Sunscreen and Insect Repellent policy.

Classroom 100 did not have sufficient lighting during nap time. Note- This was corrected during the inspection when the blinds were opened to allow adequate lighting into the classroom.

The director is missing training in the required topics of Administering Medications and Building and Premises Safety.

Two caregivers were allowed to have direct access to the children prior to having their background check ran. Note- This was corrected during the inspection when the two caregivers were sent home until the results of their background checks are received.

The first aid kit was missing tweezers and cotton balls. Note- This was corrected during the inspection when the missing items were added to the first aid kit.

Five out of five caregivers who have been employed for more than a year did not have annual training in the required topics of Administering Medications and Building and Premises Safety.
