One caregiver who the director stated received orientation during hire did not have documentation of the verified orientation. This was corrected during inspection with the director and caregiver signing completion of orientation.

Loose-fill material at both playground structures was not at the required depths. Exposed rock was observed at slide exits.

A caregiver had children place their noses to the wall during a timeout session. The timeout was in excess of the allowable time of one minute per age of the children.

The gas leak test was past due.

The smoke detectors in the 4yo room and older infant room were tested and determined to be not working.

The annual sanitation inspection was past due.

Two caregivers did not have complete CPR training that included the hands-on skills portion.

A caregiver was observed on video recording using a personal phone during different parts of the day while supervising children. The caregiver appeared to be viewing videos or scrolling through social media.

A caregiver stated during an interview that she attempted for several minutes to try and get a child to sleep when the child did not want to stay on their nap mat.

The children's products certification was past due. This was corrected at inspection by operation staff reviewing the CPSC website for recalls and certifying after no recalled items were identified at the operation.

The carbon monoxide detector was tested and not working. This was corrected during inspection with a new carbon monoxide detector inslalled.

The operation stated that fire drills for February through May 2024 were conducted, but they were not documented.

During the investigation, it was determined that the infant classroom was out of ratio at the time of the incident.

A caregiver was observed grabbing and pulling a child when redirecting for behavior.

A caregiver was observed on a personal phone for reasons not related to the direct care and supervision of children.

A caregiver did not use positive, encouraging statements and demeanor when verbally redirecting a child.

The loose-fill at the playgrounds was below the required 6 inches.

The annual certification for reviewing each of the recall notices issued by the CPSC was over 12 months.

4 child immunization records on file were not up to date.

One employee had access to children in care prior to the operation receiving a background check result of eligible, eligible with conditions, or provisionally eligible. The employee was a caregiver observed with infants.

Based on observation and interviews, it was determined that children were not engaged in routine outdoor activities when weather permitted.

Screen time activities were not being included on written activity plans.

Based on inspection observations and interviews, it was determined that the director is maintaining overall compliance; however, during the investigation inspection, it was determined that the director had a caregiver present without a background check, a caregiver who had not been providing outdoor activities on a routine basis, and screen time activities were not being included on written activity plans.
