Medication was not in the original container so the epi pen lacked vital information including the name of the child's full name who the epi pen belongs to for one child with doctor provided label.

The CPR and First Aid training for the primary caregiver expired 8/30/2025.

One child in care did not have a food allergy plan in file that required a plan in file by the doctor.

Two children did not have the date of admission listed on their file, they did not have anyone listed on the people of release by the parent or any acknowledgment, and they did not have a parent signature as required on the admission information. **The date of admission was corrected at inspection by the primary caregiver when it was written in.

The primary caregiver provided false documentation of a health statement when another child's health statement was altered to put another child's name.

A child file whose child admission date is 9/30/2020 and still does not have a health statement in file which is required 12 months from the date of admission.

A staff that was hired on 8/29/2022 did not have proof of orientation in file for review with required signatures.

A child file whose chlid admission date is 9/30/2020 did not have a health statement in file which is required 12 months from the date of admission.

A caregiver whose training year is 8/23/2021 - 8/23/2022 was lacking 3 hours for their 24 required clock hours of annual training.

Four child files were reviewed at inspection. Two children files were missing the emergency contact address for the individual listed by the child's parent. Two children files were missing the parent signature acknowledging the enrollment information provided and one of those files did not have the section for authorization for emergency medical attention, the additional information section providing information of the child's food allergy, and did not have the health statement section of what the parent will do regarding the health statement for their child.

Five children did not have cots, nap mats, or bed to sleep in and were laying on blankets folded in half.

The use zone for a swing observed being used by children did not meet the required use zone as it did not extend twice the height of the vertical distance from the swing beam to the surface below and only measured about 2-3 feet away from the fence. Additionally, a stationary equipment with a slide attached was observed without the necessary use zone of 6 feet all around.

The operation decreased the size of the outdoor play area used by the children in care prior to notifying to Child Care Regulation.

There was only one caregiver present at the start of the inspection with 8 children. One child was 13 months old. Seven children were 18 months old to three years old, putting her out of ratio by one child. This was corrected when an assistant caregiver arrived at the daycare.

There were 13 children present at the operation and the total capacity permits 12 children total.

Two caregivers did not have a high school diploma or equivalent on file. One caregiver did not have an affidavit of employment on file.

Two children currently enrolled did not have signed health statements from a medical professional.

Two infants, both ten months old, were permitted to sleep with a blanket in their pack-in-plays. One of the infants was also given a pillow with which to sleep.

Water was not made available to children during their lunch period. Vegetables were not cut into small, manageable bites for children under 2 years old. This presented a possible choking hazard.
