The operations last gas inspection was completed 3/2024.

The operation was not following conditions on an approved variance. Per varaince condition an individual is to work in a classroom with a qualified caregiver. At inspection, observed the caregiver supervising three children 6 months to 11 months alone. This was corrected at inspection as a second caregiver was sent into the classroom.

Observed two children were each napping on a torn nap mat that had exposed foam in one classroom. Nap mats cannot be properly sanitized if torn and have exposed foam.

The operational policies were evaluated and lacked written procedures for the following: A) The benefits of physical activity and outdoor play; B) The duration of physical activity at your operation, both indoor and outdoor; C) Each setting in which your physical activity program will take place; D) The criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play; and E) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

A sampling of children records were evaluated. One child's record lacked a health statement.

In one classroom, during nap time, one caregiver was supervising 7 children, ages 15 months to 17 months. These numbers exceed the ratios allowed in a Licensed Child Care Center. This was discussed with the director. This was corrected at inspection as a second caregiver went into the classroom to assist supervise the children.

One caregiver was caring for 5 infants birth through 11 months. This was corrected at inspection as a second entered the room. In another classroom one caregiver was supervising 6 children ages 14 months to 17 months. This was corrected at inspection as the second caregiver returned to the classroom.

This standard was reviewed as part of an investigation and found deficient. It was determined an individual has been working in a classroom with children as a caregiver who does not have a high school diploma or equivalent.

Diaper changing mats in two classrooms were observed torn with exposed foam which causes the mat not to be able to be properly sanitized. This was corrected at inspection as the operation replaced both changing mats.

A sampling of children records were evaluated. Seven children records did not have a signed Parent's Rights form on file for review.

One child's food allergy plan was not signed by the parent.

A child was observed seated in a highchair without straps and not strapped in. Two other high chairs did not have straps.

Three employees lacked a notarized affidavit. Two employee's record lacked documentation of an employee statement regarding the center policies. Two employee's records lacked a statement regarding the personnel policies. One caregivers record lacked documentation of completion of orientation.

A sampling of children records were evaluated. One child's immunization record was observed incomplete.

In one classroom a caregiver was supervising 25 children, ages 4 years to 6 years (SPA: 5 years). This was corrected at inspection as 3 children were moved into another class. In another classroom, one caregiver was supervising 16 children, ages 3 years. These children had woken up from nap and were putting their nap mats away. This was corrected at inspection as another caregiver went into the classroom. These numbers exceed the ratio allowed in a Licensed Child Care Center. This was discussed with the direcctor.

In one classroom, a caregiver was observed talking on her cell phone.

The diaper changing mat in one classroom was torn and had exposed foam. This was corrected at inspection as staff replaced the changing mat.

This standard was reviewed as part of an investigation and found deficient as a caregiver has put children in the corner as a method of discipline.

This standard was reviewed as part of an investigation and found deficient as a caregiver used an improper restraint which led to bruising of a child.

A sampling of children records were evaluated. One child's record lacked a health statement.
