A caregiver that has been employed longer than 90 days did not the remaining 15 clock hours required for pre-service that was hired 9/22/2025.

On the playground there is a gap between the dirt and where the bottom of the concrete walkway starts that poses a tripping hazard. Additionally, there is a jagged edge on the other side of the concrete walkway that is exposed along the bottom edge. There is a cut on the metal fence that poses a hazard to the children as it can cut them. The various ointment and suncreens were observed in the children's cubbies have been placed out of reach.

A caregiver was observed changing multiple diapers, while wearing gloves, and did not remove the gloves and wash hands after each diaper change.

A child was left on the playground outside by themselves. Staff did not have physical proximity to the child nor did they have auditory or visual awareness of the child's ongoing activities as they were unaware of where the child was.

One staff was observed caring for 8 children with one child being 16 months of age and the remainder of the children being 18 months to 4 years of age. The maximum number of children to be supervised by one caregiver with one child that is younger than 17 months of age is 5 children that are 18 months to 4 years of age and therefore, the classroom was over by 2 children in that group.

A 5 month old was observed having the bottle propped.

Infant feeding instructions were not available for review for three infants in care.

During the inspection, two caregivers were observed caring for a total of 13 children. One of the caregivers was caring for an infant younger than 12 months and 7 children that ranged from 18 months to 4 years of age. If the child that was under 12 months of age was moved to the nursery room, then that caregiver would be caring for 5 children with the specified age group being 0-11 months of age and that classroom would be over by one child.

One child file reviewed did not have any immunizations in file.

A cabinet was observed that could be opened and it had chemicals and cleaning supplies that were accessible to children.

One child in care did not have the emergency contact address provided by the parent for the individual listed as an emergency contact. Additionally, one child in care did not have the emergency medical authorization information section complete in file.

Multiple children were observed having their diaper changed and not washing hands after.

The diaper changing surface have not been sanitized appropriately after diaper changes.

A three month old infant was observed sleeping swaddled in a play yard.

A caregiver was observed doing multiple diaper changes and not washing hands after.

One caregiver was observed in direct care of 11 children with three of those children being ages 0-17 months of age; the maximum number of children to be supervised by one caregiver was 7, therefore the caregiver was over by 4 children.

The caregivers previously cited for lack of documentation of first aid & CPR had it available at inspection for review. However, a different caregiver was observed at inspection in a classroom by themselves, supervising a group of children, and lacked First Aid & Cpr training.

The operational policies did not have a complete emergency preparedness plan when reviewed.

Ointments were observed in the infant classroom that did not have the child's name on it.

A staff file was not able to be reviewed upon request and trainings were not able to be reviewed.

The play area used by children was observed having rusted screws and metal pieces as well as a wooden piece with a rusted nail sticking out of it that posed a hazard to children playing outside.

There were at least three people that are no longer associated with the operation for longer than 7 days and have not been inactivated by the operation. *This was corrected at inspection when individuals were inactivated from the operation's background check list.

The operation failed to practice all required emergency practices for 2023. Additionally, for 2024, the operation has not practiced all required fire drills monthly or monthly checks.

There were at least three people that are no longer associated with the operation for longer than 7 days and have not been inactivated by the operation.

A classroom was observed coming in from outside and not washing hands. *This was corrected when the children were instructed to wash hands in the middle of storytime.

Two children files did not have current immunization files when reviewed.

Three caregivers did not have proof of current CPR training and First Aid training in the file upon request.

During the inspection, an individual was present and had direct access to children in care prior to notification of eligibility being received.

A staff file was not able to be reviewed upon request and trainings were not able to be reviewed.

A slide was observed being used by children without the required use zone free of other objects of 6 feet.

The most recent Children's Product Certification was not available for review upon request. *This was corrected at inspection when it was completed by the person left in charge.

The operational policies did not have a complete emergency preparedness plan when reviewed.

An individual was present and in direct care of children without a background check being submitted and no letter of eligibility in file for the individual present.
