During the investigation the operation was found to be deficient with this Minimum Standard. Interviews conducted during the investigation confirmed children under the age of two years of age were being shown videos of Ms. [name redacted] and Jack Hartmann which constitutes screen time prohibited for that group age.

Caregivers were aware of that a child had recieved medicatal attention to remove a rock from the child's nose and did not report the injury to licensing. I spoke with the staff about making reports and how to make reports. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

During a DFPS investigation it was found that staff failed to review the groceries ordered online upon their delivery to ensure the items were correct to what was ordered, this resulted in peanut butter crackers being served to a child that is known to have a peanut allergy, causing an allergic reaction.

A caregiver was seen and heard yelling at a child on the playground.

There was a sink in the infant room that did not have water in it. Sink was refilled during inspection. There was no sink in room 13 for anyone to wash their hands. Director has a protable sink that she will put into classroom.

A review of the Employees Who Left Employment in 2024 data from January 16, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The operation did not have a policy for supporting inclusive services for children with special care needs.

An electrical outlet in an infant classroom was exposed. A safety cover was immediately placed on the electrical outlet when addressed, bringing the facility into compliance.

A type of equipment that children must not use was observed on the playground. The equipment was a type of jungle gym.
