Based on information gathered during the investigation and video footage reviewed, a child walked out of the gate from the playground area and was found outside on top of a vehicle in the parking lot.

Based on video footage viewed and information gathered during the investigation, two children exited the school-age classrooms unsupervised. One child made it to the foyer area, and a three-year-old made it to the outer front door of the operation. Based on information gathered, caregivers were aware that the three-year-old child was still in care.

The Pre-K and School age classrooms were observed to be out of ratio during the inspection.

Information gathered during the investigation indicates that the operation did not provide appropriate supervision to children in care. During two separate incidents, children left the building without staff awareness or supervision.

Based on information gathered during the course of the investigation, a child was injured while on the playground, and caregivers did not assess the child for injuries.

Based on a preponderance of information gathered during the investigation, the director failed to ensure the operation complied with required reporting, documentation, supervision, and emergency preparedness practices. The director was aware of multiple incidents involving children leaving the operation without supervision, a child sustaining an injury requiring medical attention, and ongoing safety concerns within the building; however, no incident or injury reports were completed or available for review. The director also failed to ensure fire drills were conducted, as they are documented.

Based on the information gathered, the operation failed to report that a child broke their arm while in care at the operation and required medical attention.

Based on information gathered during the course of the investigation, it was determined that children have been allowed into the kitchen. The kitchen is an unlicensed space and has multiple hazards present.

During the inspection, the director was asked to provide incident and injury reports for multiple events, including two children leaving the operation unsupervised and a child sustaining a broken arm. The director stated that no reports were completed or available.

Caregivers with four groups did not know the number children they were responsible for during inspection.

Based on information gathered through the course of this investigation, it is determined that the director did not report to childcare regulations as outlined in the standards.

Based on information gathered during the course of the investigation, it is determined that a caregiver did not use good judgment when chasing children around on the playground with a decomposed rat.

Based on information gathered through the course of the investigation, it is determined that a caregiver was yelling at children while in care.

Based on information gathered during the course of this investigation, it was determined that the operation did not complete an incident report regarding the incident between a caregiver and student in care, resulting in the child being left without supervision.

Based on information gathered during the course of the investigation, it was determined that a child was left unattended while in care.

The operation does not have a carbon monoxide detector installed at the operation.

One child did not have a FARE form on file at the operation and One child did not have a physician signature on the FARE form at the operation.

The operation allowed a caregiver to have access to children without obtaining the results of the background check. The caregiver was sent home.

It was determined that a child's diaper was not changed in a timely manner.

Five staff are missing a signed statement that they recieved the center policies.

Two infants do not have updated feeding instructions on file.

Nine children files are missing the updated special care needs section.

One child's file is missing the health statement.

One child's file is missing the immunizations.

A caregiver was observed in the infant classroom without a background check. The caregiver was sent home and the background check was ran while at the inspection.

A caregiver did not use good judgement by not following corporates policy for head injuries by not notifying parents immediately.
