GETCAP Head Start and Early Start at Trinity

It was found that a child was left in a classroom alone when the caregivers did not do a head count or scan the room to make sure every child exited the room.

It was found that a child was left alone in a classroom for approximately 10 minutes without supervision.

The most recent fire inspection was conducted on 3/20/2023. The director has scheduled a new inspection to be completed by the fire marshal by the compliance date.

Rather than calming herself and removing herself from the situation upon admittedly becoming overwhelmed, a caregiver persisted in her aggression, leading to the maltreatment/abuse of two children in care. Another caregiver exhibited poor judgment when she failed to intervene appropriately (e.g., by failing to alert other operation staff or verbally confronting her coworker for her abusive behaviors in the moment) during a disciplinary issue at the facility.

A caregiver repeatedly hit a child in the face (e.g., 5 times) with his own hand along with requiring him to face the outside wall, and the caregiver wrapped the child?s arm around his own neck as well. The caregiver inappropriately picked another child up by her arm, shoulders and her hair to get her to sit against a wall outside. The caregiver threw a shoe at a child, and the caregiver put the child?s shoe in the garbage can as well.

A caregiver failed to intervene to prevent a child from hitting her head on concrete during a disciplinary incident at the facility. The caregiver failed to separate the children and prevent them from hitting each other. Another caregiver failed to appropriately intervene in response to the escalating aggression which led to the abuse of two children in care.

DFPS staff determined there was reason to believe that a caregiver physically abused two children in care during a disciplinary incident at the facility.
