There prek classroom has improved, but remains cluttered and disorganized with toys and art supplies stacked on top of each other that could fall on a child's head. This area is accessible to children. There is an outdoor space that contains areas of overgrown grass with numerous stickers/burrs and ant beds that is currently not used by children due to it's condition.

There are trash cans in the classrooms accessible to the children with trash inside and without a lid, making it accessible to children. There are rodent droppings under a kitchen sink cabinet and on the floor next to the sink in the dining area.

During the investigation, information obtained through interviews indicated that a caregiver would yell at children in a manner that was intimidating or humiliating to the child.

There are rodent dropping found under the sink in the kitchen and 2 droppings found on the floor by the sink in the dining area.

A caregiver was present at the operation caring for children in the toddler classroom with children aged 23 months to 3 years old without an approved, eligible background check. The caregiver immediately left the operation.

There was an electrical outlet in the infant room that did not have a plug cover.

A caregiver changed a diaper and did not wash hands afterwards. The caregiver was prompted and then went to wash her hands.

There is a trash can in the outside area accessible to the children with trash inside and without a lid, making it accessible to children. There are rodent droppings under a sink inside the cabinet in the kitchen.

There has not been any drills conducted since 3/13/2025 for Sheltering/severe weather, 5/29/25 for fire drills, and 5/29/25 for CO Alarm detector, smoke detector, and Fire Extinguisher test. A fire drill was conducted during the inspection to bring this standard into compliance.

During an employee file audit, this document was missing from an employee file.

There is not an up to date annual fire inspection.

A child's immunization record had not been updated.

A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

The last documented fire drill occurred on 3-13-25. This was immediately corrected during the inspection by completing a fire drill. The exit time was 33 seconds.

There is a child in care with a diagnosed food allergy that did not have an emergency action plan.

There was a mop bucket with water inside the girls bathrooms. The children were also served hotdogs that were not cut up appropriately. This was corrected during the inspection.
