During the inspection, it was found that one of the dirvers for the operation had not completed their annual transportation training.

During the inspection outlets accessible to children in both the two-year old class and three-year old class were not covered by child safety covers. The Director did immediately locate new outlet covers to cover the outlets when the issue was brought to her attention.

The operation's back building suffered major damage during a January 2026 winter storm that rendered the building unsafe for human occupation. The operation did not report this until late February of 2026. The operation has now permanently closed the building.

Based on information obtained during a DFPS investigation it was determined that the director of the operation had knowledge of concerning behavior by a staff person and continued with that person's employment, even going so far as to rehire them after terminating them previously.

Based on information obtained during a DFPS investigation it was determined that children in care were abused by daycare staff. It was determined that multiple children in care had photos obtained and utilized to create explicit content.

This standard was evaluated as part of an investigation an found deficient. A child received medical attention by a health care professional when dirt was accidentally thrown into the child's eyes. The operation did not report the incident within the required time-frame.

During inspection, an infant classroom was observed using wading pools during water activities. There were seven infants observed, with the youngest being ten months old, and three caregivers present.

Based on information gathered during the investigation, the operation's staff and director have been taking children through an unlicensed space, into the director's office for separation when "acting out." The space presents a number of hazards to the children. There is broken tile through the entrance to the space and building materials.

Based on information gathered during the investigation, the director/permit holder has been separating children from the group for more than one minute per age.

Infant A and Toddler classroom's were both observed to have infants in seats without using the safety straps.

One caregiver is missing the following required topics: Emergency preparedness; Preventing and controlling the spread of communicable diseases, including immunizations; Administering medication, Preventing and responding to emergencies due to food or an allergic reaction; Understanding building and physical premises safety, and Handling, storing, and disposing of hazardous materials.

The operation has three children records that do not have signed statements of policies received on file.

A caregiver in the Two's class was observed not washing their hands following diapering. A caregiver in the Pre-K classroom was observed not washing their hands prior to handing food.

An 18 month old child in the Three's classroom does not have admission information on file at the operation.

The operation was observed using a YouTube video with visual aid during nap time in a multiple classroom space.

The one has one child has been enrolled at the operation over one year without a health statement. Another child's admission packet health statement section was completed.

Infants and Toddlers were observed being placed at a table to eat snack, without hand washing.

The operation did not notify regulations of changes to the infant room.

Two infant's written feeding plan have not been signed and updated by a parent/guardian.

The Infant II/ Toddler classroom had a specified age group of 12-17 months with six children in care and one caregiver.

Caregiver in the mixed group classroom did not know how many children were in care, after transitioning from the restroom.

Two infants in the one year old classroom were observed sleeping in their play yard with training cups in their mouth. CI: The caregiver removed both cups during inspection.

The operation's one year old classroom specified age group was 12 - 17 months. There was one caregiver and seven children present. CI: The person in charge placed another caregiver in the classroom during inspection.
