There are no sign in/out sheets on file for two children in care.

In the bathroom used by children there was a shaving razor accessible to children in care. Additionally there were multiple nails protruding from the fence in the backyard in the area that is used by children.

Ammunition is being stored in a drawer of a cabinet that is in a room where the door was not locked.

The water temperature of the sink being used by children in care reached to 161.9 degrees Fahrenheit. **This was corrected at inspection when the provider turned off the hot water for this sink.

A bottle of glue was observed within reach of children that has instructions on the bottle that say it should be stored away from children.

A child in care watched Television for over an hour.

It was observed during inspection that children were left outside alone multiple times and the caregiver did not have adequate supervision as they were unable to have visual or auditory awareness of each child. Additionally, the caregiver was not in close proximity to the children outside to be able to intervene if necessary.

The inspection form for the inspection conducted on was not posted as required. Additionally, the Permit Renewal Acceptance Letter was not posted as required.

Two infants in care did not have proof of the safe sleep for infants policy being provided to parents at enrollment.

The fire drills have not been documented since October 2023. The severe weather drill and lock down drills did not have the required drills practiced and documented at least four each in 2023. The testing and checking of smoke detectors, carbon monoxide and fire extinguishers was not documented.

An infant in care was not strapped into an equipment, rocker, that has safety straps. **This was corrected when the provider strapped the child in to the equipment.

Two infants in care did not have infant feeding instructions avaiable for review upon request.

The bathroom used by children was observed having items within reach of children that state keep out of reach of children as well as there were medicine and other items that should be kept out of reach of children.

An infant in care did not have current infant feeding instructions available for review upon request.

There operational policies did not include information to parents regarding infant safe sleep.

The primary caregiver was missing 2 hours of the required 30 clock hours in the annual training year for 12/2021 - 12/2022. **This was corrected at inspection when the provider moved over 2 1/2 hours from the current year to the last completed training year.

During the inspection children were observed playing on play equipment that was on concrete and did not have required use zone all around the equipment of at least 6 feet.

Hand washing did not occur for a child in care after having a diaper change completed.

A child was observed being placed in a high chair and eating without being strapped in.

The sign in and out logs for the last three months were not able to be reviewed at inspection.

The documentation of safety drills including checking of fire extinguisher, carbon monoxide, and smoke detectors was not available for review at inspection.

The operational policies does not have a policy or information regarding the promotion of physical activity.

The last children's product safety commission form available for review was signed on 1/5/2021. **This was corrected at inspection when it was signed by the provider at inspection.

Infant feeding instructions were not available for review for an infant in care.

Upon file reviews, one child's file had no immunizations in file for revew and another child's file did not have an up to date immunization record in file.

Upon file review of four children's records the following were found missing information: One file did not have information provided by the child's parents regarding a person of release. One file missing the address for the emergency contact listed by the parent. Two files were missing the name of physician and other pertinent information under the authorization for emergency medical attention section. One child did not have any signature under the admission requirement for a health statement.
