One child file was not available for review at inspection.

The fire inspection is past due as it was due by 4/4/2026.

Two children were observed not strapped in while sitting at a table eating that requires them to be strapped.

Based on the evidence obtained during the investigation, staff were not aware of a child's on-going activity and did not have visual or auditory awareness of a child that they were responsible for, when a child was left unsupervised in a classroom and caregivers were not aware that the child had been left behind in the classroom alone for at least 15 minutes.

Based on the evidence obtained during the investigation, the staff did not complete the incident report form that was required to be completed when a child was placed at risk when left unsupervised in a classroom alone.

A caregiver did not have proof of CPR and First Aid training when the file was reviewed.

Infants were observed seated in a table with bucket seats and there were not straps on the chairs, therefore, the infants were not strapped in with the buckles.

Two staff did not have proof of orientation completed within 7 days of employment.

An infant classroom did not have any current infant feeding instructions available for review when requested.

Four staff files reviewed and were missing the Form 2985 and one other staff file reviewed was missing the Form 2912 that is required.

Three caregiver files were reviewed and there was no proof of pre-service in file, including pre-service for staff that are working with children that are younger than 24 months.

Written activity plans were not available for two classes when requested.

A caregiver employed did not have all 24 clock hours required for the training year of 5/22/2024 - 5/22/2025 as the caregiver was missing 4.5 hours of training.

One staff file reviewed did not have all the required training topics in file as they were missing the following training topics for the previous complete training year: understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and the training for handling, storing, and disposing of hazardous materials.

Two staff did not have proof of orientation in file when reviewed. **This was corrected at inspection when the Personnel Record form was completed during the inspection.

A staff was observed in the classroom during inspection and did not have the required 8 hours of pre-service in file prior to being in the classroom and in direct care of children.

Based on the evidence obtained during the investigation, an employee inappropriately disciplined their biological children while in care by swatting their child on the side of the head.

Based on the evidence obtained during the investigation, an employee has been inappropriately disciplining biological children while in care when biting and pulling hair to show how it feels.

Based on the evidence obtained during the investigation, numerous incidents occurred related to inappropriate discipline in which may have placed a child at risk occurred and Child Care Regulation was not notified within the 48 hours of the incidents occurring.

Based on the evidence obtained during the investigation, the operation did not adequately notify a parent of a situation that placed children at risk when the children were inappropriately disciplined by caregivers over approximately a two-month period of time.

Based on the evidence obtained during the investigation, a caregiver disciplined a child by pulling the child's hair.

Based on the evidence obtained during the investigation, staff were not aware of a child's on-going activity and did not have visual or auditory awareness of the child that remained outside in the playground area and was found unsupervised outside by another adult.

Six of the files reviewed were missing information in the file which included: notarized affidavits, high school diplomas, hire dates, and proof of orientation/pre-service as well as receiving policies.

The last available menu was dated for September 2023.

Two children's files were reviewed and were missing the most current shot records in file.

CPR training reviewed for all staff was online and not vaild. One staff did not have current CPR/First aid training available for review.

One child did not have a health statement and has been enrolled more than 12 months.

One caregiver did not have any annual training to review for the training year of 3/25/2023 - 3/25/2024. One caregiver was missing four hours for the training year of 9/17/2022-9/17/2023.

A caregiver failed to use good judgement when eating a hot food in a class that injured a child.

The operation could not provide an accurate tracking system to show when children arrived and when they were picked up.

9 of the 10 admission forms did not have the emergency contact persons address.
