The food allergy plan for a child in care did not have the required doctor signature.

A classroom was observed and a child did not have their hands washed after their diaper was changed.

Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for after the rest of the class was taken in from outside. The child was left alone, outside on the playground, for approximately 10-15 minutes. Additionally, the child was discovered by another church staff employee who made the operation aware that the child was missing and was outside unsupervised.

Based on evidence obtained, the operation did not notify parents immediately of a situation that placed their child at risk when their child was left outside for approximately 10-15 minutes without a caregiver present. They notified the parents approximately 3 hours after the incident occurred where they should have been notified once the child was safe.

An infant was observed sleeping with a bib on during an observation conducted by a DFPS investigator.

Based on evidence obtained a caregiver was unable to see the ongoing activities of children, and a child had minor injuries due to another child.

A group of children were observed participating in screen time during nap/rest time during an observation conducted by a DFPS investigator.

Children's files were reviewed and there were seven children that did not have the emergency medical authorization complete in file. Three children's files were missing information for the emergency contact listed and one child was missing phone numbers for the people of release provided by the parent. Three children's files did not have the required parent signature on the child admission form.

Three classrooms did not have activity plans available for review at inspection.

Two staff did not have orientation in file as required for review.

Based on the evidence obtained during the investigation, an individual has been frequently and regularly present with direct access to children without an eligible background check notification.

The annual product certificate was not updated.

A caregiver grabbed a child and caused bruising.

DFPS investigates and determines that a staff member at the operation is found Reason to Believe for abuse, neglect, or exploitation of a child in care.

A caregiver failed to use good judgment when she grasped a child by the upper arm when directly the child to a table and chair for re-direction that resulted in an injury that required medical treatment.
