During interviews with children, parents and other educators, all expressed seeing teachers on personal devices while supervising children both in the classroom and outside on the playgrounds.

This investigation found that the incident report was not shared or signed in 48 hours.

All five operation staff interviewed verified that the ladies help in the baby room. When asked for certificates showing they have training in shaken baby syndrome, they were unable to show documentation.

All five operation staff members interviewed corroborated the allegation that the kitchen staff are helping in the baby room and stepping into the classroom to relieve teachers for breaks. When asked for proof of pre-service training and orientation, no certificates were produced.

A 17 month old child was left on the playground for 3-5 minutes.

The incident report was signed but not dated by the parent.

The caregiver watching the class during naptime had a 5-10 minute long conversation with another caregiver who came in to see if she needed a break. During their 5-10 minute conversation, there was an incident where a child inappropriately touched another child's private areas. This was witnessed on the school's webcam video.

This incident happened on 5/2/2025 and brought to the attention of Administration Staff on that date. It was called in anonymously on 5/27/2025.

An incident report was created and signed by the operation on 1/13/2025, but parents did not sign it until 1/16/2025. This is more than 48 hours later as per minimum standards.

The operation has been conducting severe weather and lock down drills twice yearly.

There was a child that was left in the hallway unattended for a few seconds, before being rejoined with their class outside.

There was a lack in supervision when two children exited the building, and emergency gate allowing them to be at the front of the building unsupervised for a few seconds.

There was a child that was left outside unattended for 2 minutes.

Based on the investigation, and video footage it was determined the operation was not in-compliance.

A child was injured when a piece of furniture that holds dress up clothing fell on a child when they tugged on the clothing hooks. The daycare removed the piece of furniture from the classroom and removed other ones like it from other classes in the building.

A caregiver did not provide proper supervision when they did not intervene in a situation to ensure a child's safety. The child was injured and had to seek medical attention.
