The food allergy plan was not available for a child in care who the parents had listed had a diagnosed food allergy.

During the inspection, children were observed playing on a play structure that required a minimum of 6 feet all around the structure and use zone and structures were observed in the use zone.

The children were observed having morning outside time but did not have drinking water available to them while outside.

The children's belongings were observed stacked on top one of another and did not have individual belongings for each child in care.

The operation was out of ratio at the start of the inspection, as there was only one staff present and there were a total of 8 children present with two of those children being 17 months and younger of age and the remaining six children were between the ages of 18 months to 3 years of age. The requirement for the maximum number of children that one caregiver may supervise in a licensed home with two children being 17 months and younger is five children between the ages of 18 months to 3 years of age with only one caregiver. *Compliance was obtained when a child was picked up by the parent.

Four child records did not have information in the file regarding the child being able to swim without assistance. **Three children's files were corrected at inspection.

An infant in care did not have a current infant feeding instructions completed by the parent. **This was corrected by the parent at the inspection.

The operation did not have the following items posted as required; the most recent inspection report form, Keeping Children's Safe Poster, Permit Renewal Application Acceptance Letter and List of Employees.

The bathroom was observed having items within reach of children that state keep out of reach of children as well as there were medicine and other items that should be kept out of reach of children. Additionally, the play room used by the children had outlets that were not covered and the fence was falling towards the backyard in the backyard.

One child file was reviewed and the child's date of admission was 8/17/2020 and there was no health statement in file and this is required to be in file by 12 months of admission date.

The drills were evaluated and the provided had only documented that they had practiced one severe weather drill and one lock-down drill for the 2022 calendar year and four of each drills have to be practiced and documented. Two other drills were documented but not specified which drill was practiced/documented.

A child file was reviewed and there were no immunizations in file to review.

Two children files were reviewed and no health statement was found in file despite being enrolled more than 12 months.
