The written notification to Child Care Regulation was not completed by the anniversary date of the issuance, or by 6/30/2025. **This was corrected at inspection when it was completed by the operation.

The water temperature in the sinks used by children exceeded 120 degrees Fahrenheit.

Broken glass was found in the play area during the inspection.

A staff file was reviewed for a staff that is providing care to children and the staff file had CPR training in file that had been received online.

A staff employed did not have all of the additional required topics in the annual training year as they were missing the following topics: building and physical premises and preventing and controlling the spread of communicable diseases, including immunizations and reacting to food allergies.

The first aid kit was not complete as there were no waterproof, disposable gloves available. **The provider corrected this at inspection when gloves were brought by a parent and placed in the first aid kit.

A staff employed did not have all of the additional required topics in the annual training year as they were missing the following topics: administering medication, if applicable, preventing and responding to emergencies due to food or an allergic reaction; and understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic for training year 6/28/2023-6/28/2024.

A child in care did not have information in file from the parents regarding the emergency medical authorization and information about food allergies. **The operation corrected this when the parent completed the form and this was available for review and it is now in the child's file.

An individual has been residing at the operation without an eligible notification from the background check unit.

There were only 17 hours avaiable for review for the primary caregiver for the last training year, 2021-2022, therefore, 13 hours were not able to be reviewed. For the other staff, there were only a total of 4 hours that were able to be reviewed during the inspection for annual training year 2021-2022.

An individual has been residing at the operation without an eligible notification from the background check unit.

The operation was out of ration at the start of the inspection, as there was only one staff present and there were a total of 10 children present with two of those children being 17 months and younger of age and the remaining eight children were between the ages of 18 months to 3 years of age. The requirement for the maximum number of children that one caregiver may supervise in a licensed home with two children being 17 months and younger is five children between the ages of 18 months to 3 years of age with only one caregiver. *Compliance was obtained when the second staff arrived to the operation.

An infant in care was not strapped in to the high chair as required. **This was corrected when the provider strapped the child in to the high chair.

The use zone for a stationary equipment was observed and the use zone for swings on the stationary equipment did not meet the required use zone as it did not extend twice the height of the vertical distance from the swing beam to the surface below and only measured. Additionally, on the stationary equipment the slide attached did not meet the necessary use zone of 6 feet all around.

The active play equipment in the back play area is not used per manufacturing instructions for the children that are permitted to play on it based on the ages of the children that can play on the equipment.

Based on the evidence obtained during the investigation, the operation increased the size of the outdoor play areas used by the children in care prior to notifying Child Care Regulation.

Based on the evidence obtained during the investigation, the operation has allowed children that are younger than five years of age to play in the front yard. The front yard is not fenced in and therefore, cannot be used for outdoor time by children that are younger than school age.

A child with a listed diagnosed food allergy did not have a food allergy plan in file for review.

A caregiver was missing 5 clock hours of the required 24 clock hours for annual training from 6/28/2021 - 6/28/2022.

Two children's files were missing information in their file. One child was missing the home address/parent home address and missing the address for one emergency contact listed by the parent. The other child file was missing the safe sleep policy in file by the parent and signed by the parent, as well as was missing the parent signature at the end of the child admission record form.

The operational policies did not have a complete policy regarding information to parents about the promotion of physical activity.

The fence had a hole and an adult hand could easily fit through the hole and it posed a hazard and risk for a child as the play area was not properly enclosed.

An infant in care that is not on table food did not have infant feeding instructions available for review.

A gate of the fenced area was not properly enclosed as the fence picket had split and the bottom hinge of the door was not properly installed. The gate is a risk to children as it could splinter children, or cause a body part to become smashed or injured and if not fixed could potentially fall on a child when opened incorrectly as only one hinge is properly attached.

A climbing structure and a Little Tyke slide were observed being used by children and each structure did not have their own use zone of six feet and overlapped with one another.

Children were observed eating snack without washing their hands.
