At the time of inspection one staff did not have current health/safety training.

At the time of inspection, a group of school age children were using personal electronics for activities other than homework. Children were observed watching videos on You Tube and playing video games that were not related to any planned activity.

Upon leaving the operation it was observed that a caregiver allowed a school age child to be among a group of high school children who are not enrolled in the program.

Upon leaving the operation a school age child in care was observed to be among a group of teenagers not enrolled at the operation. Although the caregiver was in the vicinity, physical proximity did not ensure the level of awareness to prevent situational risks.

At the time of inspection there were no protective safety covers on electrical outlets in the cafeterias. Children under 4 yrs were in care at this time.

At the time of inspection emergency practice drill documentation was not available.

At the time of inspection the operation's health inspection had expired on 8/20/2025.

At the time of inspection the operation did not have proof of current liability insurance with their records. The policy on file expired 12/2024.

At the time of inspection the indoor activity areas did not have safety covers on the electrical outlets where care was being provided for preschool children.

The operation was unable to provide a copy of the most recent fire inspection report.

At the time of inspection the operation did not have a copy of the most recent licensing inspection form posted.

At the time of inspection child admission forms did not include physician or medical facility information.

At the time of inspection child admission forms did not include the section for special care needs.

At the time of inspection electronic child records were only available to 1 of 13 staff. Only 9 of 69 children in care had paper files available on site.

At the time of inspection the operation did not have an evacuation diagram posted.

At the time of inspection 1 staff did not have personnel records for review.

At the time of inspection 3 children whose admission paperwork indicated food allergies did not have emergency food allergy plans in their files.

The operation did not have a current annual fire inspection.

The operation had not completed a recent monthly fire drill. This was corrected when a fire drill was conducted during the inspection.

It was observed that children did not wash hands before eating snacks after table activities.

The operation did not have a current annual sanitation inspection.

The operation did not have documentation of monthly fire drills.

The operation did not have an emergency evacuation diagram posted.

The operation did not have documentation of annual severe weather drills.

The operation did not have documentation of annual lock down drills.
