During today's inspection, I observed a Swiffer with a chemical attached to the mop in the children's bathroom. The director removed the mop to make it inaccessible to the children

During today s inspection, I observed that the school-age playground does not have enough loose fill. Most areas only have 1-3 inches.

The fire drill from December 2024 was not conducted or documented.

5 of 10 caregivers training did not have certificates for the trainings conducted. The staff printed the certificates for the trainings and started implementing them into the personnel files.

A caregiver was seen pushing and shoving children multiple times. This included shoving a table into a child. She was walk seen roughly snatching toys away from children. The caregiver was terminated.

A caregiver is seen hitting a child on the back of the head to get him to move out of her way. The caregiver was terminated

The caregiver forces the children in a toddler to class to sit on the carpet with no activities for more than 7 minutes.

A caregiver is seen on video grabbing and harshly pulling on the arms of several children in the class. The caregiver was terminated as a result.

This standard was found deficient as part of a DFPS Investigation. The caregiver was terminated as a result of this action.

A caregiver failed to demonstrate competency when she did not supervise children at naptime. As a result, the children were able to act out with each other for several minutes.

The cots for three of the children were not a sufficient distance from each other. As a result, one child was able to repeatedly kick another child. Also, two children were able to have inappropriate contact with each other while they remained on their cots.

Two children were able to act out sexually with each other for several minutes. The teacher was in the room neglectfully supervised the children and admitted that she was busy and did not see this occur.

The caregiver was observed, on video footage, loosing self control when she grabbed a child in a rough manner. This was completed in an act of discipline when the teacher was frustrated with the child.

Video footage from a two-year old classroom shows the caregiver made the children sit on the wall next to the bathroom for about 30 minutes. This occurred during the bathroom time. The children could have been playing during this time.

A caregiver was observed on video, and by a witness, harshly grabbing a child off the ground by one arm. The caregiver handled the child in a rough manner as she carried the child across the room.

A caregiver yelled at the children on a consistent basis. This was witnessed by management, other caregivers and a classroom visitor. Instead of directing a child to an or away from an activity, the caregiver would sit and yell at the children.
