Safety straps had been removed for cleaning from a feeding table and were not yet reinstalled before placing children in the seats. As a result, a child crawled out of their seat and fell off the table.

A child's accidental play injury occurred at the playground on a play structure that was not intended for the child's age, according to the play structures manual.

Two caregivers did not use good judgement by not notifying the director of a child's fall from a play structure that was high for the child's age and was not intended for the age of the child. The incident resulted in injury. One caregiver did not use good judgement by serving a child popcorn, which is a potential choking hazard for the child's age.

Based on the information obtained, it was determined that the operation was out if ratio in the infant room on the morning of 5/11/2026. There were 9 infants to 1 caregiver until the next caregiver arrived.

A nine month old infant was in a crib with a pacifier connected to their clothing with a lanyard and clip.

Director was not present at the time of inspection, and the only two caregivers present did not know who was in charge.

The operation failed to submit a background check to CBCU for a caregiver present with children at the time of inspection.

On March 5 and 6 of 2025, the infant room had 11 infants documented in care. The room was out ratio for maximum group size by 1 infant.
