During today's inspection, I observed 4 caregivers training records from 11/2024 - 11/2025). First caregiver (NR) only have 9.5 hours. Second caregiver (MW) 16 hours, 3rd caregiver (AW) 23.5 hours and Primary caregiver only has 14 hours.

During today's inspection, the prmary caregiver only able to find 14 hours training certification for (11/2024 - 11/2025).

During the inspection, the provider could only find 2 CPR/First Aid certifications. She will text me the other 3 caregivers' certifications. On 8/24/2026, I did not receive the caregivers' CPR/First Aid certifications.

When reviewing required postings it was observed that the notice for parents and employees was not publicly visible where parents or employees could see how to access the listing of unsafe children's products.

Primary caregiver CPR/Firstaid expired on Septmeber 3, 2025.

Primary Caregiver was observed not washing their hands before serving snack.

Children where observed not washing their hands after diapering and returning to playing.

Children where observed eating snack without washing their hands prior to.

The operation did not have any documentation of severe weather drills being conducted.

Children where observed outside at a time per the activity schedule they should have been napping.

Childrens handwashing for outdoor was observed to be a bucket system.

Two children where observed using an inflatable intended for children 3 years of age and older per the manfuactuers specification.

the operation did not have any documentation of Lockdown drills being conducted.

Primary caregiver did not have the proper employee affidavit.

Caregiver was observed not washing their hands after diapering a child.

Fire Extinguisher was observed on the counter without a mount.

6 childrens records where reviewed. Two childrens records where observed to not have the required health statement by a health professional.

Three electrical outlets where observed with no covers.

The operation had not documentation of fired drills available for review.

The operation had 8 children in care at the beginning of the inspection. 1- 4 month old 1- 5 month old 1-14 month old 1- 18 months old 2- 2 year olds 1-3 year old 1- 4 year old within an hour 3 more children arrived at the operation all school age. given the ages of the children the total number of children in care this exceeds the maximum allotted per the minimum standards.

An additional foam piece cut to fit the play yard was aboserved inside a play yard with a child in it as well as without a child.

An infant was observed in a crib with a loose fitting sheet and additional foam piece as well as two one inch mattresses stacked together.

An infant 5 months old was observed in a swing for over an hour.

An infant was observed in a crib for over an hour.

A play yard was observed to have a loose fitting sheet.

The operation had 8 children in care at the beginning of the inspection. 1- 4 month old 1- 5 month old 1-14 month old 1- 18 months old 2- 2 year olds 1-3 year old 1- 4 year old within an hour 3 more children arrived at the operation all school age. given the ages of the children the total number of children in care this exceeds the maximum allotted per the minimum standards.

4 children under 2 years old where observed participating in screen time. 4 months old, 5 months, 14 months old 18 months old.

Two individuals where present at the operation during an inspection and did not have an eligibility letter.

Operation had the following 10 children in care 2 kids who are 3 month olds 1 kid who is 1 year old 3 kids who are 2 year olds 2 kids who is 3 year old 1 kid who is 4 years old with special needs 1 kid who is 6 years old given the ages of the children the total number of children exceeds the maximum allotted per the minimum standards.

Two individuals where present at the operation during an investigation and did not have a background check nor eligibility letter.

The primary caregiver left a caregiver in charge who was not qualified per the minimum standards.

The operation did not report an incident in which a child was injured and received medical attention.
