The Childrens Courtyard

At the time of the inspection the sandbox area had boards that had separated with gaps that children's limbs could get caught in and had exposed screws. There was a soccer goal net that had broken threads that created openings large enough for children's body/body parts to go through.

At the time of the inspection 4 of the 10 children's records reviewed did not have updated records.

At the time of the inspection multiple classrooms did not have plug covers as required. The director brought out a box of plug covers and put them in as we walked through the operation.

At the time of the inspection the operation could not access the staff training records due to their online system not loading.

At the time of the inspection 1 child had a noted food allergy on their enrollment form with no documentation from a medical professional.

The children continue to not go outside as declared by the activity plans.

This standard was evaluated as part of a DFPS Investigation and found deficient. A caregiver witnessed another caregiver grab and yell at children in care and did not make a report.

Caregivers were not prepared with skills and training for their employee assignments.

Parent was not notified at end of day of less serious injuries that child received.

The operation did not have an emergency prepardenss plan prepared.

The children that are transported did not have a completed and signed emergency medical authorization form on the bus.

The caregivers were unable to determine where the bead was acquired and when the incident occurred.
