Out of 5 children's records evaluated, two children's admission information was observed to be missing birthdate information in their files. Note: The primary caregiver was corrected at inspection time .

Out of 5 children's records evaluated, the admission information of one child was observed to be missing emergency medical information in their files.

The admission forms used were outdated. The admission forms do not capture the required minimum standards changes of March 2023.

There were no written feeding instructions for the infant in care.

The documentation on the safety drills did not include severe weather or lock down.

The provider received training on 03/18/2024 on CPR/AED: Adult, Child & Infant + First Aid (BLS). The documentation lacked pediatric first aid and pediatric CPR.

There was a trampoline in the backyard with an apparatus near the tramoline which may be used to climb onto the trampoline, and the netting was unzipped.

The provider completed 20 of the required 30 annual hours of training for training period 07/21/23-07/21/24.

The children's files enrolled in care does not have a written parent notification of liability insurance. It was not posted or in the operational policy.

The required training topics are 1. emergency preparedness 2. communicable diseases to include immunizations 3. administering medication 4. preventing and responding to emergencies due to food or an allergic reaction and 6. handling, storing, and disposing of hazardous materials.

There was a product whose label indicated to keep out of reach of children. It was accessible to children as it was on the ground in the outdoor play area.

The cots were not labeled with child's name or number/child assigemen map was not available. This was corrected during the inspection when the provider placed labels on the cots.

The play structure's use zone was not free of obstacles that a child could run into or fall on top of and be injured. The play structures did not have the proper use zone.

The swings did not have the appropriate use to and fro zone. The swings were set close to the fence.

The last documentation of the dogs being vaccinated was 11/10/20. They were due to be vaccinated by 11/10/21.

There was a trampoline in the backyard with removal step ladder attached, and the netting unzipped. This was corrected at the inspection when th provider removed the step ladder and zipped the netting.

The operational policies did not indicate that the operation has animals (dogs) in the home.

There were 2 children's files that lacked the health statement from a health care professional.
