The operation failed to notify Child Care Licensing about the incident.

Based on the information obtained from the DFPS investigation ,the caregivers did not prevent the children from hitting, pushing, and kicking each other during the incident.

Based on the information obtained from the DFPS investigation , a caregiver was holding a child?s wrist to prevent her from hitting her or the other children. The caregiver failed to maintain supervision of the children while attempting to de-escalate the situation.

There was no written feeding instructions for the infants in care.

A pest extermination company has been scheduled for 5/16/25.

The required annual training on the following topics had not been completed by caregivers. The required training topics are 1. emergency preparedness 2. communicable diseases to include immunizations 3. administering medication 4. preventing and responding to emergencies due to food or an allergic reaction 5. building and physical premises safety and 6. handling, storing, and disposing of hazardous materials.

The annual transportation training had not been completed by the director. The last transportation training in the file was August 15, 2023.

There was no written feeding instructions for an infant.

The infant caregiver left an infant unattended to open the front door. The infant caregiver walked out of the infant room and went to the toddler classroom. Both times, there was no other caregiver in the infant classroom.

A roach was seen in the infant classroom. The director's spouse indicated he has exterminated. The spouse does not meet the requirement to exterminate.

Of the children transported today, only one had the emergency medical authorization in the transportation folder. This was corrected at the inspection when the director copied the medical authorization from the admission forms.

A caregiver stepped out a classroom to obtain items from another classroom/area of the operation. This left the other caregiver in care of 13 children with a specified age group of 2 years out of child/caregiver ratio by 2 children.

A blanket was in the crib of an infant, who was asleep. The caregiver removed the blanket when the inspector informed her.

A fire drill was not documented as practiced during the month of March.

The emergency medical authorization information for the child transported was not in the transportation binder. This was corrected during the inspection when the director made a copy of the medical authorization form to place in the transportation binder.

The television was on at the inspector's arrival in the classroom where the children were in care. There were 3 children in care under 24 months. The television was turned off when the inspector brought it to the director's attention. The television was on in dining area/school age classroom.

A child's record indicated a food allergy. The document from the physician indicated food allergies. There was no emergency care plan in the child's file.

Two children's records did not have the admission date recorded on the admission form.

One child's record did not have an emergency contact information recorded on the admission form.

Three children's records did not have the health statement filled out and signed by the parent.

One child's record lacked the emergency care authorization information.

There was no record of the pre-service training for a caregiver in care of children.

A child's record indicated a food allergy. The emergency care/food allergy emergency plan on file was not signed by a physician and parent.

The operational policies did not include an infant safe sleep policy.

One child's record did not have release of information recorded on the admission form.

There was no vaccine policy for the operation's employees.

The operation had not submitted the total number of employees who ceased working at the operation the previous calendar year by January 15, 2024.

There was no vaccine policy for the operation's employees.

There were no plan to handle transportation emergencies.

The operational policies did not include an infant safe sleep policy.
