A garbage can was observed to have no lid which allowed access to the trash inside. This was corrected during the inspection when a lid was placed on the garbage can.

A second classroom was observed during nap time ratios, where a child was observed under the age group. This was corrected when a second classroom combined and the child was moved to another classroom bringing the room into compliance with 10 children and two caregivers.

Another incident was observed when a caregiver stepped out of the room, leaving one caregiver with 22 children.

The one classrooms were observed out of ratio. One classroom was observed to have 5 children, bringing the room out of ratio by 1 child. This was corrected when a second caregiver entered the room.

The two classrooms were observed out of ratio by one child each. This was corrected when two children were moved to other classrooms, bringing both rooms into compliance.

Staff providing transportation, did not have transportation training, medication, or responding to medical emergencies due to food or allergic reaction. This was corrected when the caregiver took the required transportation training.

The first aid kit in the transportation vehicle was missing scissors, tweezers and a thermometer. The items were added to the kit, Bringing the kit into compliance.

A group of children were left unsupervised during the rest period.

The age group for the room is 15. The room was out of ratio by 9 children.

During the inspection, first aid supplies was observed. The kit was missing supplies and had items that were expired.

During the inspection, the operation failed to follow CBCU guidelines for employees. One staff was observed alone and providing care for children. This was corrected when another employee came in to work. The employee did not have conditions.

Children's records were evaluated and the following items were missing: health statements, parent signatures, allergy statements for children with diagnosed allergies, and immunizations.

During the inspection it was found that the infant cribs did not contain fitted sheets.

During the inspection a caregiver was observed changing a diaper but did not wash the child's hands.

During the inspection it was found that the fire drill had only been done for the month of September and November.

The changing pad in the infant classroom contained a hole.

This was corrected at inspection as another caregiver went into the toddler classroom and the director went into the school age classroom.

There were two caregivers present and directly working with children without cleared background checks.

During the inspection it was found that the operation did not have a fire extinguisher or first aid kit in one of the vehicles used to transport children in care. This was corrected at inspection when a fire extinguisher and first aid kit was placed in the vehicle.

During the inspection it was found that the written feeding instructions have not been reviewed and updated every 30 days.
