The discipline used by a caregiver with a child in care did not demonstrate acceptable behavior. By hitting the child with the hand, the caregiver did not use self-control.

A caregiver responded to a child in care in a negative way, by hitting the child after the child displayed a behavior concern.

A caregiver used physical punishment on a child in care. The caregiver hit the child on the back.

The directors had not completed the required 30 hours of training. 16 hours in additional topics related to the care of children was missing. The second director was missing 5 hours.

This standard was found deficient as part of a DFPS investigation.

During a DFPS investigation, a caregiver was found to have picked up a child from their cot and slammed them back onto their cot resulting in the child sustaining an injury near their eye that required medical attention and liquid stitching.

A caregiver was observed not washing the hands of a child after diapering. The caregiver washed the child's hands, correcting the deficiency during the inspection.

A caregiver hired after September, 2023, did not have a pre-application affidavit. This was corrected during the inspection when the caregiver was provided the form to complete.

Marked cleaning supplies were observed on the hand washing sink in the infant room. This was corrected during the inspection when the caregiver removed the cleaners.

6 children records were observed missing these items: field trips, swimming, special needs, admission dates, signatures and dates, emergency information.

Out of four staff files reviewed, three had not completed their 24 hours of pre-service training within 90 days of employment.

The operational policies were missing procedures for inclusive services to children with special needs, enrollment and policy change procedures, criteria the daycare will use to decide when weather is too extreme for outdoor play, a plan for physical activity to occur indoors when weather prohibits outdoor play, that employee vaccinations aren't required, and the DFPS website.

None of the infant feeding instructions had been updated monthly.

Out of the four staff files reviewed, none had a notarized employment affidavit on file.

The children's product recall certification form had not been signed annually.

Out of 10 children's records reviewed, one was missing a statement from the parent that operational policies had been received, three needed updated immunization records, one was missing information on whether or not he had allergies, one was missing a hearing and vision screening, two were missing emergency contacts, one was missing an admission date, and two were missing a health statement.

The director, who also transports children, did not have a certificate for the required two hours of transportation training available for review.

During the inspection a caregiver did not wash the hands of the children after diapering.

During the inspection a non mobile child was observed asleep tummy down on the floor. This was corrected when the child was picked up and placed in a crib facing up.

During the inspection, a caregiver was present without a cleared background check. This person was observed to leave the operation.
