The Learning Lodge II

During the inspection two sinks inside of the classrooms were observed without soap. This was corrected at the inspection when the caregiver filled the soap dispensers located by the sinks in the classrooms.

During the walk through two trashcans were observed without lids on them accessible to the children. This was corrected at the inspection when the director placed lids on the trashcans.

The bottles and cups were observed to not have the child's name on them.

The diaper changing mat in the infant room was observed with a hole in it. This was corrected during the inspection when the changing mat was replaced with a new one.

A fire inspection has not been conducted at the operation since March 2025.

A 15 year old was observed at the operation without an active background check.

During the inspection, a child was observed receiving a diaper change without washing his hands afterwards.

Two of the caregivers counted into ratio and alone with children did not have cpr or pediatric first aid training.

During the investigation, 3 employee files were reviewed and found to be missing a copy of the employees identification, their diploma, and the affidavit was not notarized.

During the investigation inspection, a caregiver was observed changing a child's diaper without wiping down the changing table mat afterwards. A wall plug was observed missing in the preschool room.

Milk was observed stored in a cabinet in the older infant classroom. This was corrected at the inspection when the caregiver removed the milk from the cabinet and another caregiver stored the milk in the refrigerator.

The small playground does not have two exits.

The outdoor playground area was observed with hazards. Part of the metal fence was not fully installed and presented a hazard on the smaller playground.

Two of the diaper changing mats were observed with rips and tares. This was corrected at the inspection when the diaper changing mats were swapped out.

The building was observed with dirt and dust on the floors, dirt on the walls.

A caregiver with a provisional background check was observed alone with a group of children.

A caregiver was observed changing a child's diaper without washing their hands or the child's hands afterwards.

The director only has 18 annual hours of the 30 hours required.

There were cracked ceiling panels in the preschool room and trash on the playground area.

There are no fire drills documented since December 2024.

There was 1 caregiver to 5 infants making them out of ratio.

The directors cpr and first aid credential is expired.

During the monitoring inspection it was found that the operation did not have medical release forms on the transportation vehicle for the children who are transported. This was corrected at inspection when this information was placed in the vehcile.

During the monitoring inspection it was found that there was enough loose fill to cover the entire use zone area of a slide and a climbing structure.

During the inspection it was found that the operation's transportation vehicle did not have an electronic child safety alarm installed.

During the monitoring inspection it was observed that the children in the 1 year old classroom were watching television.

During the inspection it was observed that the outdoor areas did not have loose fill where the slides, rocking equipment, or climbing equipment were.

During the inspection it was observed that it was 72 degress and sunny outside, however the children had not been outside the day of the inspection. The playground was not muddy and the play equiptment was not wet.

During the inspection it was observed that the bathroom in the school age classroom had cleaning supplies on the floor and on a low shelf. There was also clorox wipes on a shelf in the room next to the snacks that the children were being served.

When I arrived at the operation there were 9 infants, ranging from 5 months - 18 months, with no caregiver present in the room.

During the inspection it was observed that there was no paper towels in the bathroom for the school age children.

During the inspection it was observed that there were pack n plays that had a mattress that did not fit wall to wall, was not designed for the pack n play it was on, and there was added foam padding between the mattress and fitted sheet. This was re-cited.

During the inspection it was observed that there was a changing table in the pre k room with no sink in this area.

During the inspection it was observed that there was a swing in the toddler playground that was attached to a play structure.

During the inspection it was observed that there was a 5 month old baby with a bottle propped up with a blanket.

During the inspection it was observed that a caregiver sat down to feed an infant their bottle and did not wash their hands first.

During the inspection there was a caregiver with a provisional background that stated they could not be alone in ratio, this caregiver was alone with a group of 9 children, ages 2-4 years old when I arrived at the operation.

During the inspection it was observed that there was a 5 month old infant in a pack n play with a blanket.

During the investigation inspection it was observed that a caregiver was changing diapers, with gloves on, however wasn't washing their hands after taking the gloves off and before putting new ones on.

During the investigation inspection it was observed that there was pack n plays that had a mattress that did not fit wall to wall, was not designed for the pack n play it was on, and there was added foam padding between the mattress and fitted sheet.

During the investigation inspection it was observed that a child did not wash their hands, and was not assisted on washing their hands, after their diaper was changed.

During the investigation inspection it was observed that the floor in the school age room was not in good repair and due to this not able to be effectively sanitized.

During the investigation it was found that the operation was using forms of prohibited punishment by having the children do wall sits, and other forms of exercise, as a way to control their behavior.

During the investigation it was found that a person was present at the operation, in a caregiver role, with a background that stated they were not eligible to be present.

During the investigation it was found that there was a child in care who was allowed to use cleaning supplies at the operation.

Observed 11 infants in a classrooms with 3 caregivers. The specified age for the group of children in that classroom is 0-11 months. The maximum group size for 0-11 months is 10 children. This was corrected when a child was removed from the room.

During the inspection there was standing water observed in multiple riding toys in both the toddler and Pre-k playground areas. There was liter observed on the Pre-k playground.
