A caregiver did not use good judgment when grabbing up a child by the arm.

A caregiver grabbed a two-year-old child by the arm in a non-malicious way, to remove the child from the group of children. The grab was hard enough to dislocate the child's arm.

3 of the children's records did not have health statements signed a physician.

During the investigation, sufficient evidence was found to show that older infants were being combined with children over 18 months older than them. This was corrected during the inspection when the older infants were moved into their own room.

The operational policies were issing procedures for parents to review minimum standards and the most recent licensing report, contact information for the local licensing office, the DFPS website, information for nursing mothers to breastfeed children while in care, and the center's emergency preparedness plan.

The center did not have the required emergency telephone numbers printed. This was corrected when the director printed out a list of emergency telephone numbers.

Out of eight children's files reviewed, one was missing a parent's signature and healthcare facility information, three were missing updated shot records, and one was missing a health statement.

Out of three staff files reviewed, two were missing a copy of their highschool diploma, one was missing a copy of their photo ID, and one was missing a statement saying they had received personnel and operational policies, that they had reviewed policies on abuse and neglect during orientation, and a hire date. This was corrected during inspection when the missing paperwork was printed out or completed by each caregiver and placed in their file.

The center did not have written activity plans for the toddlers and pre-k groups. This was corrected at the inspection when the director created and posted schedules for each groups.

The first aid kit in the operation's transport vehicle did not contain a pair of scissors. This was corrected at the inspection when the director placed a pair of scissors in the first aid kit.

There was a toilet brush in an unlocked cabinet under the children's sink, a caregiver's backpack on the floor in the infant room within reach of children, a cracked toy and loose fence boards on the playground, and a loose outlet plate in the toddler room. During the inspection, the caregiver put the backpack up out of the reach of children and the director removed the toilet brush from under the sink, removed the broken toy from the playground and secured the loose outlet plate with tape.

The posted menu did not reflect substitutions that were made to the food for lunch. This was corrected when the director wrote in the correct foods on the menu.

The vehicle that the operation uses for transportation did not have a fire extinguisher inside. This was corrected at inspection when an extra fire extinguisher was placed inside the vehicle.

During the inspection, piles of gathered leaves, grass, and various sized rocks were observed on the large playground. The toddler playground had an outside opening that would allow rodents to enter and drop pest droppings that can cause health problems and illness.
